DUTIES AND RESPONSIBILITIES: The primary responsibility is coordination and provision of clinical pharmacy services to patients in the Community Living Center (CLC). The Clinical Pharmacist Practitioner, Pharmacy Department, applies clinical knowledge and skills when performing pharmaceutical functions and patient care activities in the Pharmacy and Medical Center. Incumbent serves as a provider with prescribing privileges as outlined in the individual's Scope of Practice for Clinical Pharmacy Practitioner (CPP). The primary responsibility of the Clinical Pharmacy Practitioner is the provision of evidence-based pharmaceutical care services through both interaction with patients and providers. The pharmacist monitors and manages drug therapy for patients referred with chronic disease states (hypertension, heart failure, diabetes, lipid management, anemia, anticoagulation, etc.). The incumbent provides pharmaceutical care utilizing an advanced in-depth knowledge of current literature and contemporary practice of medications. This pharmacist promotes and manages drug therapy based on current clinical knowledge that is consistent with policies established at the national, VISN, and local levels. The scope of this practice includes patient teaching, quality assurance, staff education and consultation, and is often in association with or in support of pharmacists, providers, nurses, dietitians, and other health care practitioners. The incumbent will serve as a clinical pharmacist specializing in Pharmaceutical Drug Management. Responsibilities include participation in the pharmacological management of patient medication needs, provision of educational services to staff and patients, the development and management of clinical programs such as drug usage evaluations and adverse drug reactions, involvement in medical center committees, and ensuring that effective clinical pharmacy services are being provided. This incumbent will serve as a provider with prescribing privileges as outlined in the individual's scope of practice. The primary responsibility of the Clinical Pharmacy Practitioner is the provision of evidence based pharmaceutical care services through both direct and indirect interaction with the patients and providers. This pharmacist promotes and manages drug therapy based on current clinical knowledge that is consistent with the policies established at the national, VISN and local levels. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. GS-13 Grade Determinations: Experience: In addition to the basic requirements, you must possess one-year of experience equivalent to the next lower grade level (GS-12). Assignments. Candidates at this grade level are to be in one of the assignments listed below. For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time. Clinical Pharmacy Specialist. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Pharmacists assigned to this position must demonstrate the following KSAs: a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. b. Expert understanding of regulatory and quality standards for their program area. c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
