The incumbent, a professional Community Care Program Coordinator-social worker, is the Community Care Programs Coordinator (CCPC) for the Geriatrics and Extended Care Service, responsible for administering, planning, monitoring, and evaluating the of the Community Care Programs, (Contract Nursing Homes, Home Respite, Homemakers/Home Health aide Program). Also, collaborates in the Veterans Directed Care Program (VDC), Hospice and Skilled Services as needed.
This professional Program Coordinator-SW coordinates a variety of activities; program evaluation, consults assessment, and modification; field onsite inspections, placements, care coordination, consultation/education to medical center staff and community service providers; the collection and maintenance of statistical data.
Screens and process consults/referrals to guarantees timely access to services by evaluating GEC Community Care Consult following the VA Policies and Requirements and Delegation of authority (DOA).
Addresses and process walk-ins' requests providing education to patients/families, caregivers, when appropriate. Process referrals and perform home evaluations visits when needed to the GEC community care programs candidates for service.
Responsible for providing follow up monthly visits to Contract Nursing Homes (CNH) in Puerto Rico and US Virgin Island, to assure service and staffing adequacy, discharge planning process and after care plans when needed. Maintains liaison with community public/private health and social service agencies, assisting patients and/or their family/caretaker, to secure those needed services not provided at this VA agency.
Monitors Community Agency provider contracts to assure compliance with the terms and conditions of the VA provider agreement. Monitors the agency's agreement technical performance requirements to assure that performance is strictly within the scope of the VA Provider Agreement.
The program coordinator will act as an advocate when it serves the best interest of the veteran/family. Acts as an advocate for the need for VA services in specific geographical areas. At all times the incumbent will represent the VA in the community in a positive and professional manner.
The GEC Community Care Program Coordinator (CCPC) SW will enter all veteran/family contacts in the electronic record using appropriate formats and templates. This information will be entered in a complete, confidential, and professional manner to ensure information on the patient is shared with other VA staff. This information will be reviewed on a regular basis.
The Program Coordinator-SW will work closely with the ACOS of Geriatrics and Extended Care to identify improvement for the patient enroll in the GEC Community Care Programs. The incumbent will participate in committee assignments, as deemed necessary by the service manager.
Work Schedule: Monday - Friday, 7:30 AM - 4:00 PM
Telework: May be available as determined by the service/facility
Qualifications

Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may NOT be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates.
GS-12 Social Worker Program Coordinatorcandidates must have the following:
Experience and Education: One (1) year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.

Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
Ability to supervise multidisciplinary staff assigned to the program.
Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
Ability to provide training, orientation, and guidance within clinical practice.
Preferred Experience: Two years experience as a Social Worker Case Manager.

The full performance level of this vacancy is GS-12.
Physical Requirements: The work may require assisting physically disabled veterans. Day-to-day work involves sitting, walking, standing, and carrying light objects, such as medical records. Work is typically performed in an office setting. Occasionally the Community Care Coordinator will visit patient home or contracted companies, for which a valid license is required. In these visits employee can be exposes to the communicable diseases common in these environments. In these occasions when the work is performed outside the medical center campus the environment cannot be controlled and safety universal precautions need to be taken by the VA personnel.
