The incumbent is the Coordinator of the Homemaker/Home Health Aide (H/HHA), In-Home Respite Programs, Veterans Directed Care and Contract Nursing Home (CNH), part of the Non-Institutional Care (NIC) Programs, for the Caribbean Healthcare System (VACHS). The VACHS offer a combination of VA-provided, VA-purchased, and VA-arranged home care services, coordinate care across settings and payer sources to ensure health care continuity for veterans.

Duties

The incumbent is the Community Care Programs Coordinator (CCPC) for the Geriatrics and Extended Care Service, responsible for administering, planning, monitoring, and evaluating the of the Community Care Programs, (Contract Nursing Homes, Home Respite, Homemakers/Home Health aide Program). Also, collaborates in the Veterans Directed Care Program (VDC), Hospice and Skilled Services as needed. Duties and responsibilities include, but are not limited to: The incumbent coordinates a variety of activities; program evaluation, consults assessment, and modification; field onsite inspections, placements, care coordination, consultation/education to medical center staff and community service providers; the collection and maintenance of statistical data. The incumbent is designated the contact person for medical center staff, and community agencies regarding community programs services and care coordination as to ensure appropriateness and continuity of care in the community setting. The incumbent will provide consultation on programs eligibility and coordination, after- care coordination, individual case management, case consultation, community agencies inspections, training, patient/family and caregiver need assessment and evaluation, program development and implementation, care brokerage, and community services linkages. Screens and process consults/referrals to guarantees timely access to services by evaluating GEC Community Care Consult following the VA Policies and Requirements and Delegation of authority (DOA). Addresses and process walk-ins' requests providing education to patients/families, caregivers, when appropriate. Process referrals and perform home evaluations visits when needed to the GEC community care programs candidates for service. Responsible for providing follow up monthly visits to Contract Nursing Homes (CNH) in Puerto Rico and US Virgin Island. Maintains liaison with community public/private health and social service agencies, assisting patients and/or their family/caretaker, to secure those needed services not provided at this VA agency. Complies with agency directives and procedures, maintains, and prepares reports, correspondence and documents as required in the medical records. Participates in continuing education programs, in quality assurance, research activities and serve as preceptor for social work trainees when appointed. Travels independently in the community to visit CNHs, and GEC Skilled and Non-Skilled community agencies/programs. Monitors Community Agency provider contracts. Serves as COR (Contract Officer's Representative) for the Contract Nursing Home (CNH), Homemakers/Home Health Aid and Home Respite Program. Serve as a liaison between contracting Homemakers/Home Health Aide, Respite Agencies and Department of Veterans Affairs (VA) Medical Center. Responsible for assuring that the corrections of deficiencies found during initial and annual inspections are made in an efficient and timely manner. Prepares monthly CNH reports and presents corrective actions when applicable. Participates as a member of the Geriatrics and Extended Care Administrative Committees. Prepares administrative and statistical reports required by the program and service complying with established policies and procedures. Assure that the quarterly Satisfaction Surveys for Homemakers/Home Health Aide, CNH, Skilled Services, Hospice and Home Respite participants be completed. The program coordinator will act as an advocate when it serves the best interest of the veteran/family. The incumbent will participate in committee assignments, as deemed necessary by the service manager. Other duties as assigned. Work Schedule: Monday - Friday, 7:00am - 4:30pm Recruitment Incentive (Sign-on Bonus): Not Authorized Pay: Competitive salary and regular salary increases. Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year). Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66. Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement) Telework: May be available as determined by the agency policy. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may NOT be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates.GS-12 Social Worker Program Coordinatorcandidates must have the following: Experience and Education: One (1) year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Day-to-day work involves sitting, walking, standing, and carrying light objects, such as medical records. Work is typically performed in an office setting. Occasionally the Community Care Coordinator will visit patient home or contracted companies, for which a valid license is required. In these visits employee can be exposes to the communicable diseases common in these environments. In these occasions when the work is performed outside the medical center campus the environment cannot be controlled and safety universal precautions need to be taken by the VA personnel.
