The Pain Management Clinical Pharmacy Specialist (CPS)works with primary care providers at the main facility in Danville, IL, and the Community Based Outpatient Clinics, serving as a mid-level provider with prescribing privileges. The incumbent delivers comprehensive medication management (CMM) through direct patient care and as part of a team providing Pain Management; The CPS is a subject matter expert on pain-related committees at the local facility level and at national and VISN levels.

Duties

Obtains medication histories from patients and summarizes significant findings in the medical record and/or to the pain management team, as appropriate. Designs, implements, assesses, monitors, and documents therapeutic plans using the most effective, least toxic, and most economical medications. Participates in the daily evaluation of patients, assesses patient response to therapy and provides appropriate therapeutic, dosing, and laboratory recommendations. Orders and assesses laboratory results, making necessary recommendations and adjustments to the patient's therapy. Identifies, documents drug errors, adverse drug reactions, allergies, and patient compliance issues perfacility procedures. Takes specific corrective action per protocol. Documents and ensures timely completion of alerts, encounters, notes, clinical interventions, consults, and/or other data collection tools per facility policy. Provides medication counseling/education to patients, caregivers, etc. Provides clinical pharmacy consultations and drug information to professional staff on the pharmaceutics, pharmacokinetics, pharmacodynamics and pharmacotherapeutics of pharmaceutical products. Basic Requirements: - United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. - Must be proficient in spoken and written English. - EDUCATION: Must be a Graduate of (or pending graduation from) an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. OR Must be a Graduate of a foreign pharmacy degree programs that meet the educational requirement and able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL IBT). (DOCUMENTATION REQUIRED) - LICENSURE: Must possess (or will possess) a current, full, active, and unrestricted license to practice pharmacy in a State, Territory or Common wealth (i.e., Puerto Rico) of the United States, or in the District of Columbia.(MUST PROVIDE A COPY) May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-13 Level: You must have at least one (1) year of creditable experience (including relevant ASHP residency experience equivalent to the GS-12 grade level which is directly related to the Pain Management Clinical Pharmacy Specialist position which includes, but are not limited to: - reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; - documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; - providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; - documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; - providing oversight of technical staff in all aspects of medication distribution. Pharmacists assigned to this position must have demonstrated the following knowledge, skills and abilities (KSAs): a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. b. Expert understanding of regulatory and quality standards for their program area c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: PGY-1 AND PGY-2 in Pain Management Board Certification;Controlled Substance Prescription Authority Experience in Pain Management Clinic with scope of practice References: VA Handbook 5005/55, Part II, Appendix G15 The full performance level of this vacancy is GS-13. Physical Requirements: The work environment involves everyday risks or discomforts, which requires that employee follows safety practices and regulations. The employee works in close proximity to patients, and is therefore, exposed to communicable diseases.
