The Home Based Primary Care (HBPC) Program Director/Manager is a Clinical Pharmacist Program Manager who executes position responsibilities that demonstrate leadership, experience, and creative approaches to management and coordination of complex client service provision. Leadership is characterized as substantial and continuous responsibility for population groups or integrated programs that cross service lines and/or discipline lines and influence organizational mission and health care.

Duties

The Home Based Primary Care (HBPC) Program Director/Manager is a Clinical Pharmacist Program Manager; the program director is accountable for the management, supervision and evaluation of care for patients enrolled in the HBPC and other VA staffed in-home programs that are organizationally aligned under HBPC. The following are assigned duties involved with this position: Facilitates an atmosphere of interactive management and development of collegial relationships among interdisciplinary team members, serving as a link between Geriatrics and Extended Care (GEC) Service and all healthcare disciplines included on the HBPC teams. Responsible for directing the clinical and administrative services offered by the program to ensure that the program is in compliance with local and VHA standards and policies as well as accreditation standards for home care organizations. Responsible for the development and continued effective functioning of the interdisciplinary health care team recognizing and utilizing the skills, knowledge and contributions of each team member. Collaborates with the HBPC Medical Director for daily program management and collaborates with other HBPC Program Directors, Veteran Integrated Service Network (VISN) leadership and VA Central Office staff on a regular and ongoing basis. Responsible for the interpretation and communication of VA policies to the HBPC team and the VHA facility. In addition, the Program Director is responsible for communicating and interpreting non-VA national, state, and local home care policies and facilitates coordination and communication with non-VA care providers when necessary. Responsible for the HBPC team development and implementation of local HBPC policies and procedures to include performance improvement, patient safety, utilization review, emergency preparedness, and staff safety. Responsible for the evaluation of the effectiveness of the program and assuring effective/efficient resources are maintained. Additionally, assures the program is able to optimize cost effectiveness (budget) and patient outcomes. Collaborates with the HBPC Medical Director to manage human resources for the HBPC Program including: selecting qualified staff, orients and mentors the staff, promotes staff development and professional growth, and assures a process for ongoing staff clinical competency including the continuing education needs of program staff and the appropriate resources to meet those needs. Coordinates trainee education programs in HBPC. Responsible for managing and tracking program resources to include but not limited to equipment inventory, GSA and VA owned Vehicles. Processes, assigns and tracks referrals for HBPC and ensures timely access to HBPC and is responsible for managing the Electronic Waiting List (EWL) for HBPC referrals. Collaborates with the Business Office and Fiscal Service to ensure accurate reporting of HBPC clinical, administrative and workload data. Responsibility for implementation of the vision, mission, philosophy, team norms, and core values for the organization. Responsible to provide assistance, collaboration and subject matter expertise for a complex group of services HBPC often works with, such as Home Oxygen Programs, Home Medical Equipment and contracting services to comply with regulatory standards. Responsible for the regular review and reporting of quality metrics. Instrumental in initiating and implementing quality improvement projects. Responsible for evaluating program activities and providing follow up actions required by internal and external review organizations and accrediting bodies. As a leadership member of the Geriatrics and extended Care Service, the Program Director participates in the formulation of overall service/program policies and procedures, goals, objectives, strategic planning, cost effectiveness, and quality monitoring. Pharmacists must be proficient in spoken and written English. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 US.C. § 7402(/)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). In addition to the basic requirements listed above, the below qualification requirements must be met. Grade Determinations: GS-13 Experience: Must have 1 year of experience equivalent to the next lower grade level that includes: (Must be documented on your resume.) Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. The primary age of patients treated and cared for are normally at the geriatric level, i.e. 60 years of age or older, although occasionally there may be younger patients between the ages of 18-60 years of age that require care. The position requires the incumbent to possess or develop an understanding of the particular needs of these types of patients.
