The purpose of this position is to provide case management care to complex gynecologic patients and obstetric patients with high-risk pregnancies, including those affected by gestational or pre-existing diabetes to meet the hospital mission of excellence by improving Special Care Obstetric Clinic continuity, coordination of care and patient flow. This position is will act as the OB/GYN Case Manager. This position is located in the Division of Clinical Services, Obstetrical Care Unit, Chinle, AZ.
Duties
Develops and maintains a registry of high-risk obstetric patients and a disease-specific registry of diabetic patients. Arranges specialty appointments and procedures as indicated at other facilities. Acts as a liaison with Purchase Referred Care Services for these appointments. Obtains discharge and operative summaries and other medical records from other facilities as needed to facilitate care. Coordinates the breakfast teaching session for pregnant diabetic patients, acting as a liaison to dietary, securing appropriate space and organizing the rotation of speakers. Assists with obstetric department quality improvement activities related to outpatient obstetric services and quality of care. Coordinates and provides Special Care Obstetrics Clinic requiring use and knowledge of epidemiology, diagnostic criteria, pathophysiology, standards of care, and modes of therapy for diabetes, gestational diabetes, and impaired glucose tolerance. Provides specialized health education to patients with specific pregnancy related conditions in the form of brochures and direct effective teaching and diverse teaching processes and principles in planning, providing and evaluating optimal health care. Provides case management of complex gynecologic patients including, but not limited to patients with gynecologic cancers and urogynecologic conditions. This may include arranging specialty appointments and obtaining outside medical records.

Qualifications

BASIC REQUIREMENT(S):
Education - A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education (external link), Council on Accreditation of Nurse Anesthesia Educational Programs (external link), Accreditation Commission for Midwifery Education (external link), or an accrediting body recognized by the U.S. Department of Education (external link) at the time the degree was obtained. Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools (external link) is required for individuals who graduated from foreign nursing schools.
Licensure - For all grade levels and positions, applicants must have passed the National Council Licensure Examination (external link). In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
Experience - The work experience must have equipped the applicant with the particular knowledge, skills, and abilities to successfully perform the duties of the position. At the GS-9 and above grade level, many positions may require experience in a specialty area of nursing. Examples of specialized experience: Independently provides professional nursing care to Obstetrical patients. Provides nursing care to the antepartum, intrapartum and postpartum patients, administering critical medications, performing cervical checks for dilation, effacement, station, presenting part, position, status of membranes, and provides nursing care for high-risk obstetric patients and a disease-specific registry of diabetic patients.
Basic Requirements for the GS-11 (or equivalent) Grade Level
In addition to the above requirements, applicants must meet one of the following requirements:
Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field; or
At least one full year of professional nursing experience (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program.
