The incumbent is the PCMM Coordinator for the Primary Care Teams within the Primary/Ambulatory Care Service of the Medical Center. The incumbent works in collaboration with the Physician Supervisors, Clinic Site Managers, Providers, Nurses, Clerks and the rest of the clinic staff.
Duties
Performs work in direct support of an administrative area such as: management analysis or operating program analysis for the Primary Care Management Module package.
Maintains full responsibility and accountability for PCMM in the Primary Care setting.
Performs work in support of general management and/or program analysis functions in which knowledge of the purpose, operation, methodology and techniques characteristic of specific management/program analysis functions is required.
Enters Primary Care information into PCMM system, enrolls patients into the Primary Care teams, and assigns patients to the appropriate provider within the PCMM system.
Maintains accurate enrollment data, identifies and corrects discrepancies in the database system, and discharges from multiple or inappropriate specialty and consult clinics.
Conducts portion of organizational surveys in a wide variety of organizational activities particularly in the Primary Care setting. Obtains, organizes, files and retrieves reports and numerical data for use in preparing status reports, briefings and presentations.
Manages administrative functions associated with the PCMM Primary Care program for the facility.
Oversees patients administrative data input into PCMM. Oversees patient processing activities in the PCMM. Tracks and documents all administrative activities and requirements for the facilities Primary Care Management Module package.
Knowledge of PCMM software, Primary Care's Strategic Plan, overall impact PCMM reports have on medical center decision, medical center's overall system of providing care to veterans, medical center's and VAMC's organizational system.
Ability to extract, correlate and analyze patient encounter and provider workload data relative to Primary Care and make recommendations to management.
All other duties as assigned by Supervisor and/or listed on the position description.
Work Schedule: Monday - Friday, 8:00am - 4:30pm

Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 07/12/2024.
Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-09 position you must have served 52 weeks at the GS-07. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. You may qualify based on your experience and/or education as described below:
Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-07 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: provides day to day management oversight of all administrative work and program activities for the department. Evaluates administrative/clinical programs and issues. Acts as a liaison between the healthcare facility and it's leaders. Is an active participant on VISN level, medical center and Primary Care committees and teams. Performs analytical/evaluative work associated with program activities, OR,
Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have a Ph. D or equivalent doctoral degree, 3 full years of progressively higher level graduate education leading to such a degree, OR,
Combination: Applicants may also combine education and experience to qualify at this level. You must have an combination of specialized experience and education beyond .

Physical Requirements: Work is generally performed in an office setting. A moderate amount of walking and standing is required when attending meetings and conferences, and in visiting other offices and buildings.
