This position is located in the Quality Management Department at the Whiteriver Service Unit in Whiteriver, AZ. The position reports directly to the Quality Management Director. The work of this position is focused on the Discharge Planning and Case Management functions, which carries the coordination and implementation of a multidisciplinary process to plan for the continuing care needs of a patients following discharge from the hospital.

Duties

Total Compensation Package - Check out IHS's outstanding total compensation package for this job: Total Compensation | Pay (ihs.gov) Duties: Applies case management essential activities assessment, planning, implementation, coordination, monitoring, evaluation, out comes across the continuum of care. Maintains current knowledge of resources available within the community, maintain supply of resource material to be distributed to patients when needed. Provide support for quality management activities as assigned. Makes rounds of patient care areas to review charts for appropriateness, completeness, and timeline of documentation by providers. Makes daily rounds to meet newly admitted patients for assessment/referral of patient's clinical needs. Serves as the specialty case management manager to ensure the continuity of care for patients through all forms of care including but not limited to the Emergency Department, Family Care Unit, and Outside facilities. Maintains a system of tracking referred patients for admission to other health care facilities to facilitate the return of patients to the whiteriver Service Unit delivery area. Performs appropriate follow up on a timely basis. Coordinating (referred) care with outside vendors, contractors, care systems, and hospitals for patients transferred for services. Discharge Planning Report to medical providers about the best placement of each patient considering the diagnosis, prognosis, and services available. Utilizes Resource and Patient Management System (RPMS), Electronic Health Record (E.H.R), including but not limited to Admission/Discharge/Transfer (ADT) Package to retrieve current patient lists and perform various functions. Documents the activities of the interdisciplinary discharge planning/case management functions in the patient record appropriately and timely. Provides information to patient, family, hospital and service unit staff on resources available in the community to patients after discharge. Utilization Review Participates in PRC for consult, review and seek approval for secondary authorization request. Collaboration with the authorization staff(PRC &UR) to maintain fiscal responsibility and efficiency.

BASIC REQUIREMENT(S): Basic Requirements OPM Link: https://www.opm.gov/policy-data-oversight/classification-qualifications/general-schedule-qualification-standards/0600/nurse-series-0610/ Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Experience: The work experience must have equipped the applicant with the particular knowledge, skills, and abilities to successfully perform the duties of the position. At the GS-09 and above grade level, many positions may require experience in a specialty area of nursing. In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below. MINIMUM QUALIFICATIONS: Your resume must demonstrate at least one (1) year of specialized experience equivalent to the GS-09 grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Developing a tracking system for patient care coordination and care management across the continuum; independently plans and provides nursing care based on professional nursing practices and medical protocols or guidelines for patients; collaborating with the physicians to develop clinical protocols and guidelines that establish standards of care; providing case management for patients by coordinating services to meet patient's care goals; providing oversight to ensure compliance and appropriateness of care. OR Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field. You must meet all qualification requirements within 30 days of the closing date of the announcement.
