The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. The incumbent serves as a Social Work Program Coordinator, locally titled Homeless Program Manager, which provides a state-of-the-art continuum of services, education, and case management to veterans with complex needs in a large, highly rural catchment area.
The incumbent is based out of the VA Health Care system (HCS) and manages the off campus Community Resource and Referral Center (CRRC). The Fargo VA HCS is in Fargo, North Dakota and is a Complexity level 2 facility. The Fargo VA HCS also has a medical school affiliation with the University of North Dakota. The Fargo VA HCS serves a large geographic catchment area of 88,000 square miles. The Fargo VA HCS has 10 community based outpatient clinics located in North Dakota and Minnesota. The Fargo VA HCS has about 35, 000 Veteran enrolled in care and provided 33,877 total outpatient visits in FY 19. The Fargo VA HCS has specialty outpatient services to include, audiology, bariatric services, cardiology, dental, acute dialysis, ear, nose and throat, emergency care, endocrinology, GI endoscopy, general surgery, hematology, hospice/palliative care, hospitalists, infectious disease, infusion center, interventional radiology, mental health/behavior health, nephrology, neurology, oncology, orthopedics/hand, ophthalmology/optometry, pain clinic, pharmacy/anticoagulation clinic, podiatry, primary care, pulmonary, sleep studies, rheumatology, urology, vascular, walk-in-clinic, women's health/OB Gyn procedures and wound clinic. The incumbent serves as the subject matter expert (SME) for Fargo VA HCS for the Multiple Homeless Veteran Program Services. He/she acts as the community liaison for all Homeless Coalitions serving Western Minnesota, and all of North Dakota. He/she has the responsibility for the organization, direction, supervision and management of Homeless Programs and the oversight of approximately 20-25 staff including the coordination of a multidisciplinary team of staff serving throughout the service region as far as 500 miles from the main campus. The Homeless Program Manager supervises, 5 -GS-12 staff and 14- GS-11 staff, 1 GS-6 Staffer. This is a clinical leadership position as well as administrative position. As such, the incumbent is a consultative member to other mental health programs, contributing expertise impacting the policies and procedures related to mental health and Homeless Programs specifically.
The Homeless Program provides services to veterans in all of North Dakota, 17 counties in Minnesota and one county in South Dakota, most of the service area is highly rural. The incumbent has full responsibility and accountability and works independently on all aspects (planning, developing, organizing, and operating) of the wide range of Clinical Programs to include Housing and Urban Development Veterans Affairs Supportive Housing Program (HUD VASH), Tribal Housing and Urban Development Veterans Affairs Supportive Housing Program (HUD-TVASH), Grant Per Diem Program(GPD), Veteran Justice Outreach Program (VJO), Homeless Veterans Community Employment Services Program (CEC), Health Care for Homeless Veterans Contract Transitional Housing (HVHC-CTH), Contract Veterans Drop in Center (VDIC), Health Care for Homeless Veterans Program (HCHV), Health Care for Homeless Veterans Contract Residential (HCHVCR), Homeless Veteran Program Services, and Community Referral and Resource Center (CRRC), within Fargo VA HCS catchment areas in North Dakota, Minnesota, and South Dakota. Incumbent is responsible for the development and maintenance of policies and procedures and establishing appropriate methods for review and control of all Homeless Program activities with expertise in ensuring staff safety, attaining, and maintaining all measurement goals, and maintaining continuous service throughout the region.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
GS-13 Social Work (Program Coordinator)
Experience/Education. One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification. The Social Work Program Coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Applicants must meet the requirements for providing supervision in the state where they are licensed at the advanced practice level and should list these qualifications within the assessment narrative and their resume. Documentation is required if part of your state's requirements for providing supervision for licensure is beyond your licensure only (example: certification of continuing education credits).
Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs (document with written demonstration of KSAs must be included with application, see required documents section):
(a) Skill in coordinating and implementing one or more specialty program(s), across the facility.
(b) Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive.
(c) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
(d) Ability to collaborate with internal and external partners to further program goals and enhance patient centered care.
(e) Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action.
(f) Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget.
(g) Knowledge of organizational structure and impact on program operations.
