This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

The incumbent develops and implements the Caregiver Support program specific to his/her area of assignment and insures compliance and collaboration with the San Francisco VA Health Care System (SFVAHCS) and the Veterans Administration Central Office caregiver support program initiatives. The incumbent conducts individual and family clinical interventions and direct patient/caregiver contacts to facilitate the identification of caregiver stress, psychiatric illness, emotional distress and social needs as treatment issues. The incumbent may assess Veterans' needs for personal care services, particularly Activities of Daily Living (ADLs), supervision, protection or instruction, to complete Veteran functional assessments (VFAI) and re-assessments. The incumbent may also assist other program staff in providing clinical services, completing notes and gathering/monitoring data related to the day-to-day functioning and evaluation of the program. The incumbent participates in face-to-face or virtual contact with the caregiver and Veteran to assess function and the need for personal care services, and as needed, the development and application of directed clinical treatment interventions. The Incumbent may conduct initial home assessments as part of the application or reassessment process for the Program of Comprehensive Assistance for Family Caregivers. The incumbent may conduct ongoing wellness contacts via in-home, telephone, or telehealth interviews (VVC) to ensure the needs of the eligible veteran and Family Caregiver are met over time, and other care needs may be addressed through referrals to other VA and non-VA services, as appropriate, at appropriate intervals determined by the National and/or local Caregiver Support Program. The incumbent provides referrals to VA medical and mental health treatment, and other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs. The incumbent will utilize and assist in maintaining a data monitoring system to measure the efficacy of the program and assists in collecting and analyzing local data for reports for: local management, VISN Caregiver Support Program Leads, Centralized Eligibility and Appeal Teams, the National Caregiver Support Program, Care Management and Social Work and Office of Patient Care Services. Evaluates program progress and outcomes and utilizes data to continuously modify and improve the program, processes, and use of a standardized assessment instrument. The incumbent serves as the major consultant for information on negotiating the complex processes the veteran and family may encounter when interacting with government and community agencies. As a Senior Social Worker, the incumbent stays abreast of current federal and state legislation directed at assisting caregivers to continue to provide caregiving in an environment that is safe for both themselves and the patient. The incumbent will maintain a knowledge base of current theory and treatment modalities by reading professional journals, attending conferences, clinic seminars and teleconferences, and materials distributed through VA and private sector sources. The incumbent will collaborate with the VA Patient Aligned Care Team (PACT), mental health, and geriatric primary care clinicians to provide Veteran and staff education on caregiver issues and to obtain input on Veteran's functional needs. Coordination will include chart review, case consultation, in-person, phone or electronic communication, and training of patient aligned care team clinical social work staff. The incumbent will respond to inquiries from the Centralized Eligibility and Appeals Team and will be responsible for tracking and processing Functional Assessment consults. The incumbent will develop and maintain a SFVAHCS Caregiver educational resource library which is available both on-line and SharePoint accessible, pertinent to both clinicians and caregivers alike consisting of caregiver support information material for distribution through the various inpatient/outpatient and specialty clinic settings. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: Citizenship: Citizen of the United States. Licensure: Candidate must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Education: Master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for master's degree in social work. Grade Determinations: Social Worker (Program Coordinator), GS-12: Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
