The Caregiver Support Coordinator (CSC) is a Social Work Program Coordinator whose primary responsibilities are to provide clinical services, community outreach, resource development, continuous quality improvement activities, and evaluation/consultation and patient/staff education on caregiving issues. In addition, the CSC must be a skilled communicator capable of developing effective partnerships with local, state and federal agencies as well as nongovernmental organizations.

Knowledge to expand clinical knowledge in the social work profession, and ability to write policies, procedures, and/or practice guidelines for the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Identify areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluation. Develops and implements the Caregiver Support program specific to his/her area of assignment and insures compliance and collaboration with the San Francisco VA Health Care System (SFVAHCS) and the Veterans Administration Central Office caregiver support program initiatives. Conducts initial, annual and, as requested, intermittent assessments to assist with the determination of eligibility for Programs under the Caregiver Support Program. This includes, but is not limited to, intakes, Veteran and Caregiver assessments, Primary Care, Specialty Care and MHTC collaboration. On an ongoing basis, the incumbent will conduct individual and family clinical interventions and direct patient/caregiver contacts to facilitate the identification of caregiver stress, psychiatric illness, emotional distress and social needs as treatment issues. Professional judgement, including the knowledge of normal and abnormal behavior, is an inherent competency applied in daily interactions with veterans and their caregivers. Assess Veterans' needs for personal care services, particularly Activities of Daily Living (ADLs), supervision, protection or instruction, to complete Veteran functional assessments (VFAI) and reassessments. The incumbent may also assist other program staff in providing clinical services, completing notes and gathering/monitoring data related to the day-to-day functioning and evaluation of the program. The incumbent participates in face-to-face or virtual contact with the caregiver and Veteran to assess function and the need for personal care services, and as needed, the development and application of directed clinical treatment interventions. Conduct initial home assessments as part of the application or reassessment process for the Program of Comprehensive Assistance for Family Caregivers. The purpose of these home visits is to assess for functional and mobility needs and safety as well as to assess need for personal care services using standardized assessments--which will assist the Centralized Eligibility and Appeal Teams (CEAT) in determining eligibility. Qualifications

Basic Requirements: 1. United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. 2. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). 3. Education: Candidates must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE).Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. 4. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grandfathering Provision: May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Social Worker (Program Coordinator), GS-12 Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Physical Requirements: The physical demands of this position involve primarily walking, talking, driving, use of the telephone, etc. Strenuous physical activity is not required although the incumbent is expected to withstand pressure and stress as part of their daily activities.
