This position, located at the Oklahoma City VA HCS, Ambulatory Care Service, provides high-level operational support and management expertise as the Facility Patient Centered Management Module (PCMM) and Home Based Primary Care Management Module coordinator.

Duties

Major duties of this position includes but are not limited to... Maintains the accuracy of the information in the PCMM Web database and responds to data updates reported in VSSC Electronically creates or modifies team and position settings, assigns provider user class based on verified credentials, link the associated clinics, set maximum panel sizes, program clinical alert notification levels, and establish preceptor to associate provider links Provides operational oversight and management to the primary care and mental health patient transfer process Supervises activities performed by employees who have access to PCMM Web and performs in depth quality assurance to all actions performs Analyzes and interprets applicable PCMM and HBPCMM data to compile regular primary care panel reports which includes information regarding the capacity of all primary care providers' panels Meets regularly with facility primary care leadership and business manager to review provider panel management data, including access for face-to-face and telephone care Manages automatic inactivation's to ensure appropriateness and communication of PACT members Participates in monthly national calls with the national PCMM Coordinator to ensure alignment with national policy, receive updates, and share strong practices Work Schedule: Monday-Friday, 7:30am-4:00pm or 8:00am-4:30pm Telework: VA supports the use of telework as a way to help attract and retain talented individuals in public service, increase worker productivity, and better prepare the agency to operate during emergencies. To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 06/04/2024. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-11 position you must have served 52 weeks at the GS-09 level. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment. You may qualify based on your experience/education as described below: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade level of GS-09 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to... .knowledge and skills in applying analytical and evaluative methods and techniques to issues or studies concerning the efficiency and effectiveness of the PACT, PCMM Web, and primary care health clinic operations carried out by administrative or professional personnel or substantive administrative support functions; knowledge of pertinent laws, regulations, policies and precedents which affect the PACT program and primary care clinics; knowledge to plan, schedule and conduct projects and studies to evaluate and recommend ways to improve the effectiveness and efficiency of work operations in the PACT program and Primary Care clinics; knowledge of the PCMM, VistA, CPRS and VSSC and the ability to utilize these systems effectively in the daily operations of PACT and primary care health clinic operations; knowledge of the principles of process improvement and quality assurance in health care to implement, evaluate and coordinate quality assurance and process improvement activities for PACT, PCMM Web, and primary care health clinic operations; knowledge of management principles, organizational theory, and techniques of analysis and evaluation; knowledge of standardized clinic operations, and practices and procedures to conduct studies of clinic work processes in various organizations to identify, analyze, and recommend solutions to problems in organizational structure, staffing, administrative procedures, work processes or workload distribution; exceptional ability to design data-gathering systems and evaluate quality of information gained, analyze and manipulate data, formulate recommendations based on data gathered, and determine impact of recommendations and/or actions; and skilled in conducting interviews with supervisors and employees to obtain information about organizational missions, functions, and work procedures...OR, Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have successfully completed a Ph.D. or equivalent doctoral degree or3 full years of progressively higher-level graduate education leading to such a degree or LL.M., if related. OR, Combination: Applicants may also combine education and experience to qualify at this level. You must have an combination of specialized experience and education beyond the graduate level of education. You will be rated on the following Competencies for this position: Database AdministrationDatabase Management SystemsEducation and TrainingOrganizational AwarenessProblem Solving Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience. Note: A full year of work is considered to be 35-40 hours of work per week. Part-time experience will be credited on the basis of time actually spent in appropriate activities.
