This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Healthcare for Homeless Veterans (HCHV) Contract Emergency Residential Services (CERS) Coordinator The Social Worker in this position functions as the VA Eastern Kansas Healthcare System HCHV Contract Emergency Residential Services (CERS) Coordinator and is responsible for monitoring and coordination of services provided by community-based agencies funded under the VA CERS Program and provides coverage and oversight of Transitional/Emergency Housing model. Major duties and responsibilities of the position include: Direct Patient Care - 60%: Coordinates with CERS staff and completes following tasks as indicated. Coordinates follow-up appointments with behavioral health providers for Veterans on the High-Risk list and provides follow-up for those who fail to attend appointments. Collaborates with the Suicide Prevention Team to ensure safety of Veterans in CERS Programs. Coordinates with PACT team regarding primary care appointments as needed. Engages in coordination with community agencies, organizations, and groups to learn of services or resources available for homeless Veterans and provides current information on services and benefits available in the VA for Homeless CERS participants. Oversees the care provided and assists with coordinating care including serving as an advocate on behalf of the Veterans. Serves as the point of contact and manages consults for the Homeless Veteran's Dental Program (HVDP). Meets with Veterans in CERS programs at least monthly, and can meet with the Veteran and contract/grantee staff, individually, or by phone for case discussion on services provided. Facilitates regular complex clinical case reviews with contractors, VA, and community agency staff. Provides consultation services for mental health, substance use, health care adherence, and is an active participant in developing treatment plans and discharge planning with contractors. Identifies high-risk Veterans to the contractor and assists in coordinating care. Collaborates with the contractor regarding implementation and facilitation of appropriate psychoeducational groups for CERS participants. Provides coverage of the HCHV walk-in clinic for those that are homeless, provide homeless hotline consult coverage, and addresses facility HCHV consults as assigned. Administrative Oversite - 40%: Provides Contract Officer Representative (COR) responsibilities for this program and must be able to gain designation by the Contracting Officer as the Contracting Officer's Representative. Stays informed of best practices/innovations in provision of CERS, prepares procurement packages, and conducts market research reports to show the need for additional services. Reviews the contract Quality Assurance Surveillance Plan (QASP) and the Performance Work Statement (PWS) to ensure obligations are met. Plans, organizes, and facilitates professional meetings with Contractors, Community Agencies, and VA programs through the acquisition process. Completes professional reports for inspections and ensures the initial and annual site inspections are completed. Serves as the Certifying Official and is responsible for invoice validation and certification and serves at the Fund Control Clerk for the CERS fund control point. Assembles and coordinates the inspection team made up of Service Line Chiefs or their designee. Validates monthly vouchers and tracking tools for per diem payments to the Grantee. Monitors funding and billing for CERS and maintains all required billing information in the electronic administration files. Monitors the clinical care and assesses the compliance of the programs receiving contracting funds and per diem as described in the contract. Conducts monthly quality assurance site visits with CERS contractor, reviewing contract compliance, and providing performance measure outcomes to the Contractor. Actively participates in planning and development, advising leadership of shifting trends and making recommendations on changes. Monitors compliance with VA performance measures and effectively shapes provider patterns to improve outcomes. Complies with conflict-of-interest laws and Executive Branch Standards of conduct to avoid conflicts of interest. Complies with the provision of "Confidential Filer" regulations which are submitted to the OGC. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. BASIC REQUIREMENTS: The basic requirements for employment as a VHA social worker are prescribed by statute in 38 U.S.C. § 7402(b)(9), as amended by section 205 of Public Law 106-419, enacted November 1, 2000. To qualify for appointment as a social worker in VHA, all applicants must meet the following: Citizenship: Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grade Determinations - Social Worker - GS-12 Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. References: VA Handbook 5005/120 PART II APPENDIX G39 Physical Requirements: Traveling throughout the medical center and the community is required, as is performing activities involving sitting, walking, and prolonged standing, bending and carrying such items as books, papers, and files. In carrying out responsibilities it may be necessary for the incumbent to travel into the community where he/she conducts interviews with the Veterans, their families, representatives of community health and welfare agencies and law enforcement agencies. The incumbent must possess current driver's license and drive a government vehicle in carrying out professional duties when deemed necessary.
