This position is located at the McClellan VA Clinic in McClellan Park, CA, and is responsible for the delivery of Whole Health Coaching and skill development of the local Medical Center's Whole Health Coach program, including provision of care through coaching and promotion and education regarding Whole Health Coaching to Veterans and staff.
Duties

NOTE: This position involves a multi-grade career ladder. The major duties listed below represent the full performance level of GS-9. At the GS-7 grade level, you will perform assignments of a more limited scope and with less independence. You will progressively acquire the background necessary to perform at the full performance level of GS-9. Promotion is at the discretion of the supervisor and is contingent upon satisfactory performance, availability of higher level work, and availability of funds.
Major Duties:
Coaches support clients in mobilizing internal strengths and external resources and develops self-management strategies for making sustainable, healthy lifestyle behavior changes and more effectively managing chronic disease.
Conducts interventions that lead to improved functioning, decreased probability of deterioration or admission, and enhanced quality of life.
Contacts eligible participants for enrollment and explain the health coaching process.
Provides health coaching for Veterans in order to reduce or eliminate high-risk behaviors while increasing healthy ones.
Conducts interventions for Veterans using one-on-one, couple, or group modalities in outpatient and inpatient settings.
Conducts Whole Health Facilitated Groups.
Participates in MOVE, smoking cessation, diabetes management, hypertension clinics, mental health, and other inpatient and outpatient groups as appropriate.
Provides follow-up care through telephone clinics or secure messaging between face-to-face appointments.
Actively guides Veterans in the exploration of core elements of personal health planning through individual and group based programs.
Develops treatment plans for Veterans focused on behavioral interventions based on current scientific knowledge and research that improve quality of life and reduce risk for chronic disease, or that reverse or slow progression of chronic disease.
Coordinates care with Veterans' treatment providers, both primary and specialty care, to ensure identified goals are consistent with best practices for treatment and management of chronic illness.
Utilizes motivational interviewing and health coaching techniques and processes, learned through prerequisite health coach training, to assist members in developing a realistic plan to accomplish their goals.
Provides eligible Veterans and family members with information regarding lifestyle management including weight management, nutrition, exercise/fitness, smoking cessation, stress management, diabetes, pain management, and other appropriate conditions that are in line with the personalized health plan developed between the health care team and the patient, and in coordination with programs providing specific services when applicable, e.g., MOVE! for weight management or smoking cessation classes, if Veteran is enrolled in these programs.
Promotes the use of health coaching throughout the facility, educating internal and external customers on the benefits of a coaching approach, publicizing available resources, developing coaching clinics and groups, participating in outreach events, and serving as a champion for Whole Health Coaching.
Performs other related duties as assigned.
Work Schedule: Monday - Friday, 8am to 4:30pm

Qualifications
Individual Occupational Requirement: You must meet one of the following three Individual Occupational Requirement options for this position. A bachelor's or higher degree that included a major field of study in behavioral or social science or related disciplines appropriate to this position. ~OR~
At least four (4) years of appropriate experience in a position that demonstrates you have acquired knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. ~OR~
A combination of education and experience that provided you with knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. The quality of the combination of education and experience must be sufficient to demonstrate that you possess the knowledge, skills, and abilities required to perform work in the occupation, and is comparable to that normally acquired through the successful completion of a full 4-year course of study with a major in the appropriate field. In addition to courses in the major and related fields, a typical college degree would have included courses that involved analysis, writing, critical thinking, research, etc. These courses would have provided you with skills and abilities sufficient to perform progressively more responsible work in the occupation. Therefore, creditable experience should have demonstrated similarly appropriate skills or abilities needed to perform the work of the occupation.
In addition to meeting one of the Individual Occupational Requirement options above, you must also meet one of the following minimum qualification requirements for the GS-7 level:
GS-7 SPECIALIZED EXPERIENCE: You must have at least one (1) full year of specialized experience equivalent to the next lower grade level in Federal service (GS-5) that has provided the knowledge, skills and abilities necessary to perform the duties of this position. The specialized experience requirement for this position is experience working in a whole health or wellness coach program that included providing interventions that promote health and wellness. This experience must be detailed in your resume to receive credit. ~OR~
GS-7 EDUCATION SUBSTITUTION: One year of graduate-level education leading to a degree in behavioral or social science or a related discipline appropriate to the position and that provided the knowledge, skills, and abilities necessary to do the work of the position. Part-time graduate education is creditable in accordance with its relationship to a year of full-time study at the school attended. GS-7 COMBINATION: A combination of specialized experience and graduate-level education as described above that demonstrates your ability to perform the duties of this position. Experience must be detailed in your resume and transcripts (official or unofficial) are required. ~OR~
GS-7 SUPERIOR ACADEMIC ACHIEVEMENT: Superior Academic Achievement (SAA) at the baccalaureate level in the qualifying curriculum detailed above is fully qualifying. To qualify for SAA, applicants must have completed all the requirements for a bachelor's degree in a curriculum that is qualifying for the position to be filled and meet one of the following. Rank in the upper one-third of your class in the college, university or major subdivision.
A grade-point average of "B" (a GPA of 95 or higher out of a possible 4.0) for all completed undergraduate courses, or those completed in the last two years of undergraduate study.
A grade point average of "B+" (a GPA of 45 or higher out of a possible 4.0) for all courses in your major field of study, or those courses in your major completed in the last two years of undergraduate study.

GS-9 SPECIALIZED EXPERIENCE: You must have at least one (1) full year of specialized experience equivalent to the next lower grade level in Federal service (GS-7) that has provided the knowledge, skills and abilities necessary to perform the duties of this position. The specialized experience requirement for this position is experience providing care and intervention through coaching and education in whole health to promote health and wellness. Examples of specialized experience include assisting in determining patient goals, assessing patient progress, adjusting plans as necessary, etc. ~OR~
GS-9 EDUCATION SUBSTITUTION: You may substitute specialized experience with education if you have at least two years of progressively higher level graduate education leading to a master's degree or a master's or equivalent graduate degree in a behavioral or social science; or related discipline appropriate to the position. This education must have been obtained at an accredited college or university. ~OR~
GS-9 COMBINATION: A combination of education and experience may also be qualifying if you have an equivalent combination of successfully completed graduate level education and specialized experience, as described above, which meets the total experience requirements for this grade level. NOTE: Your experience must be documented in your resume and transcripts are required.
