This position is located at VA Northern California Health Care System - McClellan VA Clinic in McClellan Park, CA. The Social Science Program Coordinator is responsible for the delivery of Whole Health Coaching and skill development of the local Medical Center's Whole Health Coach program, including provision of care through coaching and promotion and education regarding Whole Health Coaching to Veterans and staff.

Duties

***THIS IS NOT A VIRTUAL POSITION, YOU MUST LIVE WITHIN OR BE WILLING TO RELOCATE WITHIN A COMMUTABLE DISTANCE OF THE DUTY LOCATION*** The Social Science Program Coordinator provides care to Veterans seeking self-directed, lasting changes aligned with their values and interventions that promote health and wellness and enhance well-being, improve health related outcomes, reduce likelihood of inpatient admission, and improve quality of life. Major duties and responsibilities include but not limited to: Contacting eligible participants for enrollment and explain the health coaching process; Providing health coaching for Veterans in order to reduce or eliminate high-risk behaviors while increasing healthy ones; Conducting interventions for Veterans using one-on-one, couple, or group modalities in outpatient and inpatient settings; Conducting Whole Health Facilitated Groups and participating in MOVE, smoking cessation, diabetes management, hypertension clinics, mental health, and other inpatient and outpatient groups as appropriate; Providing follow-up care through telephone clinics or secure messaging between face-to-face appointments; Guiding Veterans in the exploration of core elements of personal health planning through individual and group based programs; Encouraging and assisting Veterans with the adoption of habits that are conducive to a higher quality of life and in alignment with the Veteran's goals; Developing treatment plans for Veterans focused on behavioral interventions based on current scientific knowledge and research; Reviewing current health behaviors and develops customized treatment plans with Veterans; Coordinating care with Veterans' treatment providers, both primary and specialty care; Utilizing motivational interviewing and health coaching techniques and processes, learned through prerequisite health coach training; Fostering an environment of individual responsibility to maximize each Veteran's ability to adhere to a lifestyle management plan; Providing eligible Veterans and family members with information regarding lifestyle management; Developing information for Veterans about the Whole Health Group and Individual Programs; Conducting outreach activities to inform Veterans and family members of available programs; Providing educational opportunities for staff-including lunch and learns, grand rounds, and presentations; Meeting with external stakeholders to provide information about the Whole Health Group and individual programs and access to Veterans; Collaborating with internal and external resources for supporting the Veteran's Whole Health Journey; Developing and conducting onsite educational series for enrolled Veterans, family members, and VA employees to explain health coaching; Developing and distributing locally approved health education materials to identified enrolled Veterans and family members; Serving on Healthcare System, VISN, and/or National VA committees or task forces specifically relating to Whole Health or Health Coaching; Representing Primary Care or Patient Centered Care/Whole Health on decision-making committees within the Healthcare System; Representing the Health Care System at professional programs and/or events, or on VA or community committees or work groups addressing health coaching; Assisting with data management analyses, special studies, and reports for use by management; Maintaining a close working relationship with various program officials; Coordinating and providing recommendations in the application of standards that affect the service; Providing training and troubleshooting for other health coaches in health coaching administrative processes and associated supporting issues; Performing other duties as assigned; Work Schedule: Monday to Friday; 08:00 a.m. to 04:30 p.m. Recruitment & Relocation Incentives: Not authorized Telework: VA supports the use of telework as a way to help attract and retain talented individuals in public service, increase worker productivity, and better prepare the agency to operate during emergencies. ENGLISH LANGUAGE PROFICIENCY: In accordance with 38 U.S.C. 7402(d), No person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. You must be proficient in basic written and spoken English in order to meet the requirements of this position. -AND- This position includes a Selective Placement Factor (skill, knowledge, ability or other worker characteristic basic to - and essential for - satisfactory performance of the job). Selective Placement Factors are a prerequisite to appointment and represent minimum requirements for a position. Applicants who do not meet it are ineligible for further consideration. The Selective Placement Factor for this position is: Experience in health coaching to enhance weight management, smoking cessation, well-being and assist with management of chronic disease/pain. This position is embedded in Pain Management. NOTE: Evidence of the Selective Placement Factor must be reflected in your resume. -AND- Basic Requirement: The following are basic requirements for appointment as a Social Science Program Coordinator in the Veterans Health Administration (VHA), which all applicants must met. Basic Education: Bachelor's or higher degree in behavioral or social science or related disciplines appropriate to the position. OR, Basic Experience: Four (4) years of appropriate experience that demonstrated that the applicant has acquired knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. OR, Basic Combination of Education and Experience: Combination of education and experience that provided you with knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. (UNOFFICIAL TRANSCRIPT AND/OR CLEAR DETAILS OF EXPERIENCE REQUIRED: If you do not submit the required document, your application may not be reviewed.) -AND- Minimum Requirement: In addition to meeting the basic requirements one of the following minimum requirement criteria must be met to be qualified for this position. Specialized Experience (GS-09): You must have one (1) year of specialized experience that equipped you with the particular knowledge, skills, and abilities (KSA's) to perform successfully the duties of the position, and that is typically in or related to the position to be filled. To be creditable, specialized experience must have been equivalent to at least the next lower grade level (i.e., GS-07) in the normal line of progression for the occupation in the organization. Qualifying examples of specialized experience would typically include but are not limited to: working as a health coach applying knowledge of health coaching structure, process, skills and techniques; supporting clients' plans for health and well-being and providing information and resources for achieving goals; retrieving, tracking and trending information needed for reporting related to health coaching; and utilizing automated systems and interpersonal and communication skills. (CLEAR DETAILS OF EXPERIENCE REQUIRED: Your resume must show complete information for each job entry, such as beginning and ending dates of employment, duties performed, and/or total hours worked per week.) OR, Education (GS-09): Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have two (2) years of progressively higher level graduate education leading to a master's degree or master's or equivalent graduate degree that provided the knowledge, skills, and abilities necessary to do the work. NOTE: One (1) year of full-time graduate education is considered to be the number of credit hours that the school attended has determined to represent one (1) year of full-time study. If that information cannot be obtained from the school, 18 semester hours should be considered as satisfying the one (1) year of full-time study requirement. Part-time graduate education is creditable in accordance with its relationship to a year of full-time study at the school attended. OR, Combination (GS-09): Applicants may also combine education and experience to qualify at this level. You must have a combination of specialized experience and education as described above. Physical Requirements: : Regular and recurring work is mostly sedentary involving sitting at a desk, although the position requires the ability to move from one geographic location of the facility to another.
