This position is responsible for the delivery of Whole Health Coaching and skill development of the local Medical Center's Whole Health Coach program, including provision of care through coaching and promotion and education regarding Whole Health Coaching to Veterans and staff.

Duties

This position involves a multi-grade career ladder. The major duties listed below represent the full performance level of GS-09. At the GS-07 grade level, you will perform assignments of a more limited scope and with less independence. You will progressively acquire the background necessary to perform at the full performance level of GS-09. Promotion is at the discretion of the supervisor and is contingent upon satisfactory performance, availability of higher level work, and availability of funds. The position works with individuals and groups throughout the facility in a client-centered process to facilitate and empower the client to develop and achieve self-determined goals related to health and wellness. Coaches support clients in mobilizing internal strengths and external resources and develops self-management strategies for making sustainable, healthy lifestyle behavior changes and more effectively managing chronic disease. While health and wellness coaches do not diagnose conditions, prescribe treatments, or provide psychotherapeutic interventions, they provide expert guidance and may offer resources from nationally recognized authorities. Contacts eligible participants for enrollment and explain the health coaching process. Provides health coaching for Veterans in order to reduce or eliminate high-risk behaviors while increasing healthy ones. Conducts interventions for Veterans using one-on-one, couple, or group modalities in outpatient and inpatient settings. Conducts Whole Health Facilitated Groups; participate in MOVE, smoking cessation, diabetes management, hypertension clinics, mental health, and other inpatient and outpatient groups as appropriate. Provides follow-up care through telephone clinics or secure messaging between face-to-face appointments. Actively guides Veterans in the exploration of core elements of personal health planning through individual and group based programs. Encourages and assists Veterans with the adoption of habits that are conducive to a higher quality of life and in alignment with the Veteran's goals. Identify habits and behaviors that are harmful to quality of life and related to chronic illness. Qualifications

BASIC EDUCATION REQUIREMENT: A bachelor's degree in behavioral or social science; or in a related discipline that is appropriate to the work of this position. Transcripts required. -OR- A combination of education and experience that provided you with knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. This experience must be detailed in your resume to receive credit and transcripts required. -OR- Four years of appropriate experience that demonstrates that you have acquired knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. This experience must be detailed in your resume to receive credit. Inaddition meeting the education or experience requirement as stated above you must also meet one of the requirements listed below. One (1) full year of specialized experience equipped with the knowledge, skills, and abilities to successfully perform the duties of the Social Science Program Specialist position. To be creditable, this specialized experience must have been equivalent to the GS-7 level in Federal Service. The following qualifying specialized experience includes advising, educating, Knowledge of principles, techniques, and practices of Employee Whole Health as well as health promotion and employee wellness to plan and perform a wide range of activities related to the planning, development, and implementation of Employee Whole Health programs. Experience using a wide range of qualitative and quantitative methods for the assessment and improvement of program effectiveness; conducting need assessments of employees to determine program needs and activities. (This must be documented in your resume)-OR- Successfully completed two (2) years of progressively higher-level graduate education leading to a master's degree or master's or equivalent graduate degree in a related field of the position to be filled? Education at the graduate level must have been obtained in an accredited college or university and must demonstrate the knowledge, skills, and abilities necessary to do the work of this position. (Transcripts Required)-OR- A combination of successfully completed graduate level education (beyond the first year) and specialized experience as described above, to meet the total experience requirements? The education portion must include graduate courses that demonstrate the knowledge, skills, and abilities necessary to do to the work of this position. This education must have been obtained in an accredited college or university. (Transcripts Required) -AND- In accordance with 38 U.S.C. 7402(d), No person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. You must be proficient in basic written and spoken English in order to meet the requirements of this position. All experience listed on your resume must include the month and year start/end dates. Part-time experience will be credited on the basis of time actually spent in appropriate activities. You will receive credit for all qualifying experience, including volunteer experience. Physical Requirements: The work is often sedentary. Typically, the employee sits comfortably to do the work. However, there is walking, standing, bending, carrying of light items such as papers and books, and driving an automobile. No special physical demands are required to perform the work.
