You will be responsible for providing pharmaceutical care to assigned inpatients, long term care patients and outpatients as directed by the Associate Chief of Acute Care Pharmacy. The primary area of involvement of this position is the Spinal Cord Injury (SCI) unit in the outpatient setting through SCI annual visits and is responsible for coordinating and providing clinical pharmacy services in the SCI outpatient setting in collaboration with the SCI outpatient providers.

VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws Clinical Pharmacy Specialist, Spinal Cord Injury duties include but not limited to: Clinical Pharmacy Specialist, SCI pharmacist is responsible for conducting direct patient care activities that are performed in the outpatient setting which includes designing, recommending, monitoring, and evaluating patient specific pharmacotherapy for common chronic disease states such as (but not limited to) dyslipidemia, hypertension, diabetes mellitus, congestive heart failure, and chronic obstructive pulmonary disease, and wound care. Clinical Pharmacy Specialist, SCI pharmacist possesses anticoagulation management scope of practice, as part of collaborative medication management, to hold warfarin and other oral and parenteral anticoagulation medications prescriptive authority; order, interpret, and monitor laboratory results; develop and execute patient-centered therapeutic plans within the parameters of their scope of practice. Clinical Pharmacy Specialist, SCI pharmacist possesses scope of practice, as part of collaborative medication management, to hold prescriptive authority to perform comprehensive medication management for SCI patients including Vancomycin dosing, Aminoglycosides dosing and medication renal dosing adjustments. Prescriptive authority includes initiating, modifying, and continuing medication regimens, ordering related laboratory tests and diagnostic studies, and taking independent corrective action for identified drug-induced problems to maximize positive drug therapy outcomes. (prescriptive authority does not extend to controlled substances) Clinical Pharmacy Specialist, SCI pharmacist promotes delivering opioid overdose reversal therapy and helps reducing opioid overdose deaths by possessing scope of practice, as part of collaborative medication management, to hold prescriptive authority to furnish naloxone prescriptions to patients on chronic opioid treatment or when needed. Clinical Pharmacy Specialist, SCI pharmacist is ideally placed to advise people on how to stop smoking and to provide information on the supply of medication. The pharmacist possesses scope of practice, as part of collaborative medication management, to hold prescriptive authority to provide treatment for smokers who want to stop and coordinate with smoking cessation clinic for continuation of care after discharge. Routine clinical pharmacist duties include ordering, reviewing and analyzing appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy, pharmacokinetic monitoring, ordering medications, devices, supplies, monitoring for appropriate dosing based on renal and hepatic function, completing non-formulary medication consults for SCI patients or other departments as assigned, and consulting as appropriate to maximize positive drug therapy outcomes. Knowledgeable about pharmacokinetics, dosing and administration of reversal agents and specific antidotes for vitamin K antagonists and direct oral anticoagulants (DOAC). Responsible for verifying dosing, medication preparation or retrieval, nursing administration guidance and patient monitoring. Designs a pharmaceutical care plan if needed and document the plan in progress note. All activities and interventions performed are documented in CPRS, allowing for seamless transitions of care and effective communication within the patient's healthcare team. In collaboration with the SCI physician and the patient, the incumbent plays a key role in antimicrobial stewardship and determining antimicrobial selection. The incumbent works closely with the antimicrobial stewardship team to ensure antimicrobial stewardship initiatives are implemented and maintained appropriately. Education: Provides patient and provider education as a core member of SCI teams during the daily patient care rounds and annual SCI visits. Serves as the medication expert in the SCI patients and is available to provide time sensitive, accurate information for other healthcare providers in acute scenarios. Educates other healthcare providers on approved treatment guidelines and criteria to increase appropriate and cost-effective use of medications. Serves as the subject matter expert in SCI medical supplies and wound care products and is available to provide education to other healthcare providers. Provides educational in-services for the SCI healthcare team on pharmacy-related topics that can improve safe and appropriate medication use and enhance team awareness overall. BASIC REQUIREMENTS: Citizenship. Citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified candidates in accordance with chapter 3, section A, paragraph 3g, this part.) Education. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. GS-13 (Clinical Pharmacy Specialist) Experience: Applicants must have at least 1 year of experience equivalent to the next lower grade level (GS-12). In addition to the experience above, the candidate must demonstrate the following KSAs: Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience/Qualifications: Pharm. D. degree and completion of an accredited Post Graduated year 1 (PGY-1 Pharmacy Practice residency and PGY-2 residency in Oncology, or equivalent experience demonstrating competency). Board Certification in Oncology (BCOP) or Pharmacotherapy (BCPS) References: VA Handbook 5005, Part II, Appendix G15- Licensed Pharmacist Qualification Standard. QUALIFICATION STANDARD GS-660 Veterans Health Administration. Physical Requirements: The work requires regular and recurring physical exertion, involving standing or sitting for prolonged periods of time, typing, walking distances within the medical center, lifting/ carrying (up to 40 pounds) of pharmaceutical supplies, and bending/stooping/stretching.
