You will be responsible for providing pharmaceutical care to assigned inpatient and outpatient areas as directed by the Director, Inpatient Pharmacy. The primary area of involvement for the incumbent of this position is the Emergency Department (ED) and Interventional Radiology (IR). You will be a highly-trained medication management expert and consultant in highly complex, clinical scenarios, collaborating with providers to ensure safe, appropriate, and cost-effective use of medications in ED.

Emergency Medicine Clinical Pharmacy Specialist (EMCPS) possess a scope of practice that is being used greater than 25% of their work time for the following clinical tasks:
EMCPS possesses anticoagulation management scope of practice, as part of collaborative medication management, to hold warfarin and other oral and parenteral anticoagulation medications prescriptive authority; order, interpret, and monitor laboratory results; develop and execute patient centered therapeutic plans within the parameters of their scope of practice.
EMCPS is well-trained in chronic disease states and performs comprehensive disease management inclusive of ordering necessary labs, medications, consults, or supplies, and ensuring coordination of care with patient's primary care team that assists in transitions of care for traveling Veterans.
EMCPS is available for high-risk procedures, particularly those requiring selection of high-risk medications, such as rapid sequence intubation which involves the selection of sedative and paralytic agents to facilitate successful intubation in patients with acute respiratory failure.
EMCPS promotes delivering opioid overdose reversal therapy and helps reducing opioid overdose deaths by possessing scope of practice, as part of collaborative medication management, to hold prescriptive authority to furnish naloxone prescriptions to patients on chronic opioid treatment or when needed.
EMCPS Pharmacists are ideally placed to advise people on how to stop smoking and to provide information on the supply of medication.
Comprehensive medication management (CMM} services for a variety of acute and chronic disease states and is well-versed in their respective treatment guidelines in ED including but not limited to:
Routine clinical pharmacist duties include ordering, reviewing and analyzing appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy, pharmacokinetic monitoring, ordering medications, devices, supplies, monitoring for appropriate dosing based on renal and hepatic function, completing non-formulary medication consults for ED patients or other departments as assigned, and consulting as appropriate to maximize positive drug therapy outcomes.
Knowledgeable about pharmacokinetics, dosing and administration of reversal agents and specific antidotes for vitamin K antagonists and direct oral anticoagulants (DOAC). Responsible for verifying dosing, medication preparation or retrieval, nursing administration guidance and patient monitoring.
Interviews and evaluates patients' medication regimens before discharge from the ED and/or before admission to the hospital as assigned for accuracy and appropriateness of therapy and document medication reconciliation note in the Computerized Patient Care Record System (CPRS).
Designs a pharmaceutical care plan if needed and document the plan in progress note. All activities and interventions performed are documented in CPRS, allowing for seamless transitions of care and effective communication within the patient's healthcare team.
Actively participates in bedside procedures requiring high-risk medications or interventions, for example, stroke codes and rapid sequence intubation procedures.
Present during medical emergencies in the ED and will play a vital role in resuscitation efforts during code-blue situations. Must have the necessary training and certifications to ensure the highest level of clinical competence with Advanced Cardiac Life Support (ACLS) certification.
In collaboration with the ED physician and the patient, the incumbent plays a key role in antimicrobial stewardship and determining antimicrobial selection. The incumbent reviews culture results and susceptibility profiles and recommend initiation or changes in antimicrobial therapy when needed. The incumbent works closely with the antimicrobial stewardship team to ensure antimicrobial stewardship initiatives are implemented and maintained appropriately.

Qualifications

BASIC REQUIREMENTS:
Citizenship. Citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified candidates in accordance with chapter 3, section A, paragraph 3g, this part.)
Education.
Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.)
Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
GS-13 (Clinical Pharmacy Specialist) Experience: Applicants must have at least 1 year of experience equivalent to the next lower grade level (GS-12). In addition to the experience above, the candidate must demonstrate the following KSAs:
Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
Expert understanding of regulatory and quality standards for their program area.
Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
Preferred Experience/Qualifications:
Pharm. D. degree and completion of an accredited Post Graduated year 1 (PGY-1 Pharmacy Practice residency and PGY-2 residency in an equivalent experience demonstrating competency).

Physical Requirements: The work requires regular and recurring physical exertion, involving standing or sitting for prolonged periods of time, typing, walking distances within the medical center, lifting/ carrying (up to 40 pounds) of pharmaceutical supplies, and bending/stooping/stretching.
