Clinical Pharmacy Practitioner/Home Based Primary Care (HBPC) must be a licensed pharmacist, preferably with a Pharm.D., and have completed a residency or fellowship, or equivalent experience demonstrating competence. Duties

Functions of the Position: Clinical Pharmacy Practitioner Duties Core functions include face-to-face, VVC (VA Video Connect) and telephone encounters with referred patients, for comprehensive medication management in practice areas identified in their individual scope of practice. Functions as a mid-level provider to design, implement, and monitor therapeutic drug plans to achieve definite outcomes through direct interactions with patients and providers in assigned areas. Orders, performs, reviews, and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy. Applies knowledge of normal laboratory values in the evaluation of patient care, recognizes significant abnormalities, and makes dose adjustment recommendations based on objective laboratory findings. Appropriately renders care to their respective patients including documentation in the computer with appropriate follow-up. When applicable, all patient interviews and assessment notes are completed appropriately in a manner consistent with the current policy. Follows-up with patients on lab or test results to discuss the plan of therapy, i.e. changes in medication therapy, monitoring, and additional testing requirements. Clinical pharmacy Practitioners will determine if a referral to the physician is necessary when disease progression occurs, or adverse drug events require treatment interventions outside of the CPP scope of practice. Completes all initial and quarterly HBPC patient care reviews as assigned. Pharmaceutical Care Activities Identifies patients who may benefit from comprehensive medication management through population management reports, during team huddles and referrals. Reviews patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed. Reports drug errors, adverse drug reactions, allergies, and patient compliance issues. Documents findings per facility procedures. Responsible for acting as stewards of the hospital formulary, with particular respect to cost effective management of the use of available therapeutic agents. This includes the utilization of computerized monitoring systems combined with incumbent clinical drug knowledge to constantly monitor drug usage and make recommendations for formulary changes when deemed necessary to assure optimal usage of drug resources. Assists HBPC with safety and quality performance initiatives. Promotes and monitors compliance for established drug therapy policies. Works with providers to ensure compliance with national, VISN, and local initiatives. Serves as one of the main pharmacy representatives for HBPC initial and quarterly reviews, section meetings, retreats and other meetings as needed. Education/Training Serves as a preceptor in the training and in-servicing of pharmacists, pharmacy students, pharmacy residents, and other medical center personnel as required. Participates in self-development activities to maintain and increase professional competence. Participates in the development and implementation of formal and informal drug education/clinical consultation to medical, surgical, nursing and pharmacy staff in the form of in-services, one-on-one encounters and continuing education programs. Serves as a resource to patients, providers and other pharmacy staff. Effectively instructs patients and family members in the appropriate use of medications and medical devices. Is able to adjust communication and tracking methods based on population and developmental considerations. Is able to accurately assess and document patient comprehension. Is able to refer patients to educational programs provided at the medical center by multidisciplinary teams. Interfaces with other health care providers and administrators to ensure compliance with established pharmacy and medical center policies and procedures. Supervises and trains pharmacy residents and students. Evaluates students based on performance, presentations, and examinations. Provides in-services on ambulatory care drug therapy to students, residents and to professional healthcare staff when appropriate. Maintains an organized, structured, clinical ambulatory care rotation for pharmacy students and residents. The appointing official may waive the requirement of licensure for a period not to exceed 2 years for a pharmacist that provides care under the supervision of a licensed pharmacist. For grade levels above the GS-11, the candidate must be licensed. Failure to Obtain License. In all cases, pharmacists must actively pursue meeting state prerequisites for licensure starting from the date of their appointment. At the time of appointment, the supervisor will provide the unlicensed pharmacist with the written requirement to obtain licensure, the date by which the license must be acquired, and the consequences for not becoming licensed by the deadline. Failure to become licensed within 2 years from date of appointment will result in removal from the GS-0660 Pharmacist series and may result in termination of employment. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-13 Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Assignments. Candidates at this grade level are to be in one of the assignments listed below. For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time. Clinical Pharmacy Specialist. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Pharmacists assigned to this position must demonstrate the following KSAs: Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: 1 year of experience equivalent to the next lower grade level (GS-12), or Completion of an ACPE-accredited Pharm.D. program.
