The purpose of this position is to manage outpatient health care delivery to patients with oncologic disease and to assist Internal Medicine Case Managers and Internal Medicine Specialty Clinic Case Managers. This position is located at the Chinle Service Unit, Division of Clinical Services, Primary care internal Medicine, Chinle, Arizona.
Duties
Develops and maintains disease specific patient registries for oncology patients. Develops and maintains referral logs for oncology patients. Arranges and assists with appointments and procedures for patient at other facilities within the IHS and at contract facilities. These may include biopsies, surgeries, chemotherapy, radiation therapy and palliative care referrals. Contacts high-risks or non-adherent patients via phone, mail, and or referral. Maintains up to date access to electronic health records databases in a timely fashion. Obtains pertinent outside facility records including office notes, discharge summaries, labs, reports, procedures notes, and imagining. Assists with appropriate and timely information transmission to and from specialist. Assists with coordination of transportation, meals, and lodging for patient traveling for their care. Obtains, develops, improves and maintains educational handouts for patients with targeted illnesses. Works closely with patient education staff and physicians to ensure that adequate and accurate patients education material is readily available. Provides direct nursing care in outpatient settings. Provides health education to patients with targets illness in the forms of brochures and direct teaching. Reviews charts of patients wit targeted illnesses during their stay as an inpatient and works closely with discharge planner to coordinate referrals and follow-up. Contacts patient with test results and arrange appropriate follow-up with primary physician supervision.
Qualifications

BASIC REQUIREMENT(S):
Education - A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education (external link), Council on Accreditation of Nurse Anesthesia Educational Programs (external link), Accreditation Commission for Midwifery Education (external link), or an accrediting body recognized by the U.S. Department of Education (external link) at the time the degree was obtained. Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools (external link) is required for individuals who graduated from foreign nursing schools.
Licensure - For all grade levels and positions, applicants must have passed the National Council Licensure Examination (external link). In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
Experience - The work experience must have equipped the applicant with the particular knowledge, skills, and abilities to successfully perform the duties of the position. At the GS-9 and above grade level, many positions may require experience in a specialty area of nursing. Examples of specialized experience: developing and maintaining patient registries by arranging appointments and procedures for patients to other facilities. Providing nursing care and health education to patients with infectious diseases.
Basic Requirements for the GS-11 (or equivalent) Grade Level
In addition to the above requirements, applicants must meet one of the following requirements:
Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field; or
At least one full year of professional nursing experience (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program.
