The Program Analyst (Patient Centered Management (PCMM) Coordinator) ensures that data input in primary care systems is accurate to properly determine future needs of staff, Patient Aligned Care Teams (PACT) teams, and capacity. Serves as a health care professional who uses data to influence and adapt health care systems to drive management of the health care system toward effectiveness and efficiency within the facility clinic area to improve and sustain operations and access to care.

Duties

Major duties may include but are not limited to: Data Entry, Data Analysis, and Database Maintenance - 50% Careful data collection, monitoring and maintaining provider profiles, monitoring panel sizes, oversight and collection of transmission errors; Review/rectification of incorrect patient assignments; Prepares National, VISN and Medical Center level reports, upon request using established computer programming routines; Collects, compiles, analyze and presents data regarding the integrity of the PCMM package; Assure training, implementation; monitoring and evaluation of all administrative procedures related to PCMM operations; Serve in an administrative capacity to executive management in the section and service; Primary responsibility for establishing and managing PCMM teams and positions set up for all primary and specialty care/ PACT teams under JJP VAMC; Uses multiple assignment tools to move large groups of patients to teams and positions, when changes occur daily; Primary Data Managing - 15% Uses data to influence health care systems to drive management of the health system toward effectiveness and efficiency; Improve and sustain operations and access to care; Performs workflow assessment and optimization, and makes recommendations in clinical, administrative, educational and evaluation domains as needed; Analyzes data and prepares accurate reports on a regular and recurring basis; The Patient Care Management Module (PCMM) Coordinator for Primary Care and Specialty Care teams within the Health Care System (HCS) service area; Resource to clinical staff in troubleshooting of DHCP, VISTA and Patient Care Management Module (PCMM) software applications associated with the clinic treatment areas; Develop, analyze, evaluate, advise on, or improve the effectiveness of work methods and procedures, organization, manpower utilization, information, and documentation systems; Project Management and Reporting - 15% Assists the Group Practice Manager in projects by developing custom reports needed to develop an in-depth analysis of all Primary Care and Specialty clinics; Information obtained through this sampling of custom reports shall be used to support Senior Level decision making regarding budgetary decisions, supply, clinic needs, staffing and long- and short-range goals efficiency, access, workload, etc.; Knowledgeable and participates in the prioritization of resources; Develops and implements an effective system that promotes appropriate utilization of all resources; Runs reports of workload and ensures deviations from planned workload is detected and addressed; PCMM Station Expert - 10% Provides relevant clinical and /or administrative data to identify, and resolve organizational problems; Consults with IT to integrate software applications appropriately and for data reporting methods for clinical services; Using data bases and registries, formulates tools to support patient care and ensure quality management and clinical oversight of the access related processes; Works with Team Leaders and Clinic Managers to formulate and conduct training and orientation for all providers and schedulers involved with PCMM; Provides expertise and advice to all providers and clerks involved with PCMM; Obtains, organizes, files, and retrieves reports and numerical data for use in preparing status reports, briefings and presentations; Program Management Functions - 10% Manages administrative functions associated with the PCMM for the facility; Assists professional staff with and handles administrative functions of the office; Gathers administrative information regarding patients for managers; Manages scheduling for the facility's projects, questions or problems associated with patient processing for the Patient Care Management Module for the facility; Oversees patient's administrative data input into PCMM; Oversees patient processing activities in the PCMM; Tracks and documents all administrative activities and requirements for the facility's Patient Care Management Module package; and Performs other duties as assigned Work Schedule: Monday - Friday, 8:00am - 4:30pm Telework: VA supports the use of telework as a way to help attract and retain talented individuals in public service, increase worker productivity, and better prepare the agency to operate during emergencies. Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 03/18/2024. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-9 position you must have served 52 weeks at the GS-7. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Failure to provide an SF-50 that demonstrates one-year time-in-grade will result in disqualification from being considered You may qualify based on your experience and/or education as described below: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-07 in the Federal service in the normal line of progression for the occupation in the organization. Specialized experience is that which has equipped you with the particular knowledge, skills, and abilities to perform successfully the duties of the position, and that is typically in or related to the work of the position to be filled. Qualifying experience is experience applying analytical and evaluative methods and techniques to issues or studies concerning the efficiency and effectiveness of a clinical program; experience planning, organizing, and developing studies or projects, build and submit statistics, analytic proposed projects, reports and recommendations of data; communication skills, both written and oral, in order to prepare memoranda, letters, policies, procedures, and variety of reports; making recommendations on operating programs and negotiating with others to accept and implement recommendations; experience evaluating and making recommendations regarding health care delivery systems; and Skilled in use of PC software programs including word processing, databases and spreadsheets such as Microsoft Excel and Word to perform data management/administration functions. NOTE: Such experience MUST be well represented in resume. Failure to do so will results in a Non-Qualification Determination.. OR, Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have a master's or equivalent graduate degree or two full years of progressively higher-level graduate education leading to such a degree that is related to this position. NOTE: Your education must demonstrate the knowledge, skills, and abilities necessary to do the work of this position. One year of full-time graduate education is considered to be the number of credit hours that the school attended has determined to represent one year of full-time study; if that number cannot be obtained from the transcript, 18 semester hours will be considered as one full academic year of graduate study. Transcripts must be submitted with application and must include degree and date degree was awarded/conferred to be creditable. Education cannot be credited without documentation.. OR, Combination: Applicants may also combine education and experience to qualify at this level. You must have an equivalent combination of specialized experience and graduate education as described above to fully meet the requirements of this position. Preferred Experience: Knowledge of Practice Management and patient flow; computer knowledge in VISTA; knowledge in graphics software such as Microsoft PowerPoint. Physical Requirements: The work is sedentary in nature. Manual dexterity is required in operation of a personal computer. The work sometimes requires walking, standing, bending, lifting or carrying light items such as paper, books and charts. Travel to facility CBOCs and divisions/sites of care is required to verify/validate rooms, staffing, and primary care setup on a periodic basis.
