The Social Worker Program Coordinator is an independent Clinical Social Worker assigned to Oklahoma City VA Medical Centers' Primary Care Clinics. The incumbent will work with various patient populations to include homeless veterans, Community Living Center, Home Health services, financial resources, VA benefits, and transportation services. The incumbent must be skilled in managing therapeutic and psychoeducational groups and be capable of completing psychosocial assessments.

Duties

Screening/Assessment- The social worker will complete an assessment of the veteran in collaboration with the interdisciplinary treatment team, the veteran, family members and significant others. The incumbent demonstrates an understanding of the range of treatment and skills needed for all adult patients including geriatric patients related to their physical limitations, psychological needs and age associated illnesses and conditions. Treatment Planning/Goal Setting_- The Social Worker is responsible for contributing to the development of the treatment plan and setting achievable treatment goals with the veteran and family members/caregiver. Referral to service providers - The Social Worker will collaborate with other service providers in reassessing the veteran's needs. The Social Worker is responsible for educating the veteran and family members/caregivers of the available services and assisting them in establishing the appropriate referrals. Resource Usage/Development- The Social Worker is responsible for using the current Social Work resource file of VA and community social service programs and enhancing the content to the benefit of veterans. The Social Worker will refer the veteran to needed services. Advocacy-The Social Worker understands the intimidation of bureaucracy and will act as an advocate when it serves the best interest of the veteran and family members/caregiver. Crisis Intervention -The Social Worker is experienced in making rapid assessments and developing crisis management plans. The Social Worker has access to multiple resource directories in addition to previously developed resources to meet the demands of a crisis. Education/Health promotion and prevention -The Social Worker provides education related to VA and community resources, entitlements, Advance Directives/Living Will and will refer veterans and family members/caregiver to the appropriate interdisciplinary team member for identified health education needs. Coordination of non-institutional and institutional services -The Social Worker will review the progress notes from the other providers to accurately determine the strengths and limitations of each veteran. Supportive counseling -The Social Worker will provide the veterans and their family members/caregiver with ongoing supportive counseling. The purpose of such counseling is to deal with the psychosocial impact of coping with chronic/disabling illness (es}, onset of a catastrophic illness mental illness(es), etc. Subject Matter Expert- The position will be a subject expert for Advance Care Planning via Group Visits, understanding documentation for State and VA process. The position will be a champion for advancing conversations on Advance Care Planning. Groups-The position will complete regular groups for ACP-GV's. The position will demonstrate an understanding of group facilitation, development, and recruitment. When appropriate the position will utilize telehealth for groups. Coordination- The position will coordinate ADP-GV's development and implementation at the assigned facility and/or CBOC's. The position will provide facility education and Veteran recruitment for ADP-GV's. The position will provide training regarding ACP-GV's as requested. Develops a working relationships and agreements with other organizations having responsibilities for the same patient population. Maintains a current and productive network of referral resources. Makes provision for referral services from other agencies, community resources (i.e., transition or halfway houses, shelters, etc.), and other Governmental agencies to resolve problems. Serves as a mentor and as a consultant to other Social Workers, particularly those at the entry level. Assists in the training of newly hired Social Workers within the medicine social work section. May provide clinical supervision for unlicensed Social Workers if all requirements are met by Veteran's Health Administration and the Oklahoma Social Work Licensure Board. May act as a field instructor or preceptor, supervises Social Work graduate students and provides a training experience for them in Social Work. The Social Worker will attend all appropriate staff meetings (interdisciplinary treatment team meetings, Social Work staff meetings, Social Work supervision meetings, and other committees and workgroups, as appropriate) and perform other duties as required. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. References: VA Handbook 5005/120, Part II, Appendix G39 Physical Requirements: Incumbents must be physically and mentally able to efficiently perform the essential functions of the position without hazard to themselves or others. Normal work of the position requires visual acuity (with the ability to read typewritten characters without strain, corrective lenses are permitted), keen hearing (aid permitted), clear distinctive speech, and manual dexterity. This position requires potentially moderate levels of lifting and carrying (15-44 pounds). The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment.
