The incumbent serves as a Program Analyst (Patient Centered Management Module Facility Coordinator), located in VISN 19, Oklahoma City VA Health Care System. The incumbent of this position, is located under the Business Manager, Ambulatory Care, and has primary responsibility as the Patient Centered Management Module (PCMM), and Home Based Primary Care management Module (HBPCMM) Coordinator for all Primary Care sites for the Oklahoma City VA Health Care System.
Duties
Major duties of this position include, but are not limited to:
Assisting operating officials in implementing identified improvement actions and plans through participation in organizational improvement work through station programs such as Systems Redesign projects
Completing comparative analysis of VSSC reports or data sets with local PCMM to identify data variance and ensure data integrity
Completing multiple assignment/reassignment to move large groups of PACT populations to different teams, when changes become necessary
Delegating PCMM Web user permissions for facility staff members
Ensuring facility PCMM standard operation procedures (SOP) or Directives are in place and current; meeting all local requirements and stakeholder concurrence to meet VHA PCMM goals and objectives
Ensuring the integrity of the VA medical facility's PCMM application, its functionality, utilization, and resulting data
Ensuring that PCMM users are knowledgeable in PCMM functionality and utilization
Ensuring that PCMM Patient Aligned Care Team (PACT) pending assignments are entered prior to the patient's first PACT appointment and no later than 24 hours from the creation of the patient's first PACT appointment.
Work Schedule: Monday through Friday, 7:30am - 4:00pm or 8:00am -4:30pm

Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 03/05/2024.
Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-11 position you must have served 52 weeks at the GS-09. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade.
Specialized Experience: You must have one (1) full year of specialized experience equivalent to at least the next lower grade level (GS-9) in the Federal service with the knowledge, skills, and abilities to successfully perform the duties of the Program Analyst, and that is typically in or related to the work of this position. Specialized experience includes: integrating Patient Centered Management Module (PCMM) and Home Based Primary Care management Module (HBPCMM) Web data, reviewing PCMM and HBPCMM data related to efficiency, workload, and staffing, assists officials in implementing identified improvement actions and plans, develops designs and completes reports related to PCMM Web, Primary Care, and collects, compiles, analyzes and presents data regarding the integrity of the PCMM Web package. Note: Experience must be fully documented on your resume and must include job title, duties, month and year start/end dates AND hour worked per week. OR,
Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have successfully completed a Ph.D. or equivalent doctoral degree or 3 full years of progressively higher-level graduate education leading to such a degree or LL.M., if related that demonstrates the knowledge, skills, and abilities necessary to do the work of this position. NOTE: Transcripts must be submitted with application. Education cannot be credited without documentation. OR,
Combination: Applicants may also combine education and experience to qualify at this level. You must have equivalent combinations of successfully completed graduate level education (in excess of the first 2 years or 36 semester hours) and specialized experience to meet total experience requirements. NOTE: If using education combined with specialized experience to qualify, a copy of your transcript is required.
