The Nurse Practitioner ID is a skilled health care provider who utilizes critical judgement in the performance of comprehensive health assessments and differential diagnoses The NP ID is competent in performing anal pap smears, High Resolution Anoscopy (HRA) and other related diagnostic and treatment procedures for the ID PACT Anal Cancer Screening Program. The NP ID will report to the Chief of Infectious Disease.
Duties
He/she is further responsible for serving as the Anal Dysplasia Program Director, as well as HIV Outreach Program Coordinator. Administrative duties include day to day management of the Anal Dysplasia Program to include instruction of ID Attendings, Fellows and Residents in the performance of anal cytology collection and interpretation as well as HRA performance. The Infectious Disease NP serves as a resource to other members of the health care team, providing consultation and education as needed. He/she demonstrates leadership through development and coordination of programs to improve quality of care and organizational processes. In addition, he/she is responsible for the overall HIV screening and education outreach program under ID to include activities for staff and patients within the main hospital/ACC, and community based out-patient clinics. management of treatment for HIV infected Veterans. This role is more specifically defined and described in the attached competency based functional statements, within the framework of the nine dimensions of the Revised Nurse Qualification Standards specific to each grade. Unit-specific or Individual-specific and core-competencies further define role requirements. The APN ID is responsible for acute and follow up care of Patient aligned care team (PACT), Human Immune Deficiency (HIV), HIV Prevention (Prep) and anal dysplasia patients of the ID Section. He/she is to maintain safe evaluation and treatment of these ID patients. The APN ID is responsible for interviewing, evaluating and assessing, diagnosing and treating patients presenting to the assigned ID Clinics. The ID APN utilizes preventative clinical guidelines, appropriate ID standards of care, and collaboration with Infectious Disease Physicians in providing quality care to assigned ID patients. The scope of practice authorizes the APN to obtain informed consent and the scope of practice authorizes them to perform the treatment or procedures for which the APN obtains consent. The APN ID conducts a comprehensive assessment of patients' medical, physical and biopsychosocial status.
Obtains a relevant health and medical history
Performs a physical examination based on age and history
Conducts preventative screening procedures based on age and history
Updates evaluation of health status to determine need for further health care
Formulates appropriate medical diagnoses based on data obtained
Orders appropriate diagnostic procedures and laboratory tests
Identifies and prescribes appropriate pharmacological and non-pharmacological agents
Identifies needs of the individual and family as a result of the evaluation of collected data
Provides relevant patient/family education
Makes appropriate referrals to other health professionals and community agencies
Consults with collaborating health care professionals when doubt exists as to the diagnosis, when expected improvement is not soon apparent and when specialized therapeutic or diagnostic techniques are indicated.
Determines the effectiveness of the plan of care through documentation of client care outcomes
Reassesses and modifies the plan as necessary to achieve medical and health goals
Obtains consent for and performs High Resolution Anoscopy based on abnormal anal cytology findings.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Nurse Practitioner must be licensed or otherwise recognized as a nurse practitioner in a State, possess a master's degree from a program accredited by the NLNAC or CCNE, and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. [The certification must be in the specialty to which the individual is being appointed or selected.]
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience.
Preferred Experience:
FNP is preferred for the primary care management of these specialty PACT patients.
Minimum of one year experience in High Resolution Anoscopy w/wo Biopsy.
Minimum of one year of specialized HIV patient management.
Minimum of one year experience in anal cancer screening.
