The Home-Based Primary Care Physician Assistant or Nurse Practitioner supply medical care of patients, identifying medical problems, defining the medical management, prescribing medications and treatment in accordance with their individual scope of practice. He/She functions as the HBPC provider for patients and coordinates their care among all members of the health care team, including other staff, outpatient primary and specialty physicians, inpatient services, ancillary hospital services.

Guides and directs others who provide care. - Identifies ethical issues in practice and takes appropriate action - Delegates care in a safe, efficient, and cost-effective manner. Assists clients in identifying and securing appropriate services. - Adheres to established policies, standards of care and practice - Screen patients and conduct initial assessments for admission into the program. - Demonstrates leadership in managing/coordinating care for patients and families assuming responsibility and accountability for actions. - Completes initial assessments and reassessment of patient's condition and response to treatment and other clinical findings. Initiates follow-up as needed and documents using clear, concise and objective data. - Refers, consults and makes provisions for continuing care with healthcare providers. - Functions in an advanced practice role. - Prescribes and/or recommends laboratory tests, diagnostic studies, and/or medications based on current medical center, VHA, and DEA policy and within scope of practice. - Acts as a role model, mentor, and coach to the program's staff. Provides developmental opportunities for program staff to acquire nursing skills and expertise. - Assumes accountability for remaining current regarding standards of care established by the profession, regulatory bodies, and government; is able to determine relevant implications for a specific area and respond using flexibility and creativity to promote compliance. - Develops and participates in strategic planning for the program. - Supports and facilitates research activities in the area of Home Based Primary Care. - Screen patients and conduct initial assessments for admission into the programs. Clinical Duties: The PA is expected to remain privileged and practicing in their specialty field. The exact privileges and procedures for a particular provider will be stated in the Scope of Practice and approved by the Professional Standards Board the PA meet the following qualifications: Patient Care: Practitioners are expected to provide patient care that is compassionate, appropriate, and effective for the promotion of health, prevention of illness, treatment of disease, and care at the end of life. Medical/Clinical Knowledge: Practitioners are expected to demonstrate knowledge of established and evolving biomedical, clinical and social sciences, and the application of their knowledge to patient care and the education of others. Clinical Judgment: To ensure practice-based learning and improvement, practitioners are expected to be able to use scientific evidence and methods to investigate, evaluate, and improve patient care. Interpersonal & Communication Skills: Practitioners are expected to demonstrate interpersonal and communication skills that enable them to establish and maintain professional relationships with patients, families, and other members of the health care team. Professionalism: Practitioners are expected to demonstrate behaviors that reflect a commitment to continuous professional development, ethical practice, and understanding and sensitivity to diversity, and a responsible attitude toward their patients, their profession, and additional duties as required by leadership. Systems-Based Practice: Practitioners are expected to demonstrate both an understanding of the contexts and systems in which health care is provided, and the ability to apply this knowledge to improve optimizing health care. Work with clinic RN to triage patients; respond to telephone calls and complete paperwork requested by patients (Aid and Attendance, Disability Parking, various insurance forms, etc.). The PA will review incomplete patient encounters for all assigned clinics, in order to ensure timely completion of documentation and billing. The PA may be responsible for answering clinical consults from other services, as deemed appropriate by the collaborating physician. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Master's or Doctoral Degree from a program accredited by the Accreditation Commission for Education in Nursing or the Commission (ACEN-Formerly NCLAC) or the Collegiate Nursing Education (CCNE). Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Physician Assistant Basic Requirements Master's degree in any discipline or a bachelor's degree in any discipline with one year of experience as a Physician Assistant ~AND~ Successfully passed the PA National Certifying Exam (PANCE) (or are you in the process to taking the exam) ~AND~ Currently hold, or will hold, a full and unrestricted license to practice in a State, Territory, or Commonwealth of the United States or in the District of Columbia. Nurse Practitioners must be licensed or otherwise recognized as a nurse practitioner in a State and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. The certification must be in the specialty to which the individual is being appointed or selected. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: The full performance level of this vacancy is VN-03. The actual grade at which an applicant may be selected for this vacancy is in the range of VN-01 to VN-03. Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical inpatient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. The incumbent who provides care in home settings or other off-site locations may be required to drive and/or ride in GSA-vehicles.
