Serve as the Program Support Assistant for Home Based Primary Care (HBPC) working in the Port Charlotte Annex supporting the HBPC Teams. Working with clinical staff daily to support weekly IDT Team meetings and monitoring staffing census for program.
Duties
Major duties and responsibilities of this position include, but are not limited to, the following:
Provide administrative and technical support to the program director and all other staff.
Plan, develop and coordinate all administrative functions essential to program implementation and the delivery of services including initiation of corrective action.
Exercise appropriate independent judgement when training incoming emergency requests from field clinicians, patients and caregivers.
Manage the program information system, collect data, submit reports, and correct errors.
Refer patients to community agencies to provide homemaker/home health aide services.
Work Schedule: Monday-Friday, 8:00 a.m. - 4:30 p.m.

Qualifications
Preferred Experience in Geriatrics Home Based Primary Care and or PACT Clinic teams. Preference in working with Spreadsheets in Microsoft office.
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 02/08/2024.
Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-06 position you must have served 52 weeks at the GS-05. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. General Adjustment SF-50's with the effective date of within the last year do not provide the information to reflect time-in-grade requirement has been met and therefore cannot be the only SF-50. If you are currently serving in a Hybrid Title 38 position, your SF-50's must reflect you have been in the position longer than one year. Please ensure read guidance noted below on acceptable SF-50's. SF-50's can be found in your eOPF.
You may qualify based on your experience as described below:
Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-05 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: provide administrative and technical support; extend training in operations of computer programs; assist with evaluating efficient means of documentation; refer patients to community agencies; ensure proper administrative procedures are understood and followed by vendors; maintain statistical data; perform timekeeping duties; process admissions; maintain computer files.

Physical Requirements:This position requires sitting for long periods of time and using the computer for many hours per day. Walking to other parts of the medical center my be necessary. Some bending and standing is required.
