The CLC/ PC Pharmacist (CPS) initiates, modifies, or discontinues medication therapy as a consultant for medication therapy management; designing, implementing, assessing, monitoring and documenting therapeutic plans using effective medication treatments. The CPS interacts with patients and interdisciplinary teams; performs assessments and orders tests to determine medication efficacy and toxicity. The CPS works under the Chief of Pharmacy and the Associate Chief of Clinical Pharmacy Services.

Duties

Scans the internal and external environments to determine opportunities to enhance pharmacy services to support patient education, patient adherence, clinical outcomes, and team member growth and development. Monitors drug utilization (formulary, non-formulary, restricted) and intervenes to ensure formulary adherence and optimal evidence-based clinical outcomes. Monitors the educational needs of providers, pharmacists, and patients, and makes recommendations to address any deficiencies. Routinely benchmarks with national contemporary programs and makes recommendations for clinical pharmacy service and lead program changes when directly involved in the assigned practice area. Analyzes and interprets the published literature, and appropriately applies findings. Participates in the development of quality and performance monitoring programs to support the optimal use of our personnel and medication resources. Generates cost containment ideas and then implements and enforces departmental and/or institutional programs to support the wise use of available resources. Acts as a liaison for service line leadership, pharmacy management, medical staff and healthcare personnel, and pharmacy team members. Actively supports the development, implementation, management and monitoring of clinical and operational guidelines - national, VISN and local. Actively supports through personal and team mentoring the professional growth of assigned personnel. Actively supports the service line team-based management of the pharmacy service. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizen: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language: You must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Education: Graduate of - or pending graduation from - an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. OR Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination(FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to the Basic Requirements above, applicants must meet the following grade requirements. GS-13 Clinical Pharmacy Specialist - Community Living Center/Palliative Care (CLC/ PC): Experience:1 full year of experience equivalent to the next lower grade level, (GS-12) where you handled routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations including, but not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution. Demonstrated Knowledge, Skills, and Abilities (KSAs) Pharmacists assigned to this position must demonstrate the following KSAs: 1. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. 2. Expert understanding of regulatory and quality standards for their program area. 3. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. 4. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. 5. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: Prefer long term care experience and PGY-1 Pharmacy Residency Training References: VA HANDBOOK 5005/55, PART II, APPENDIX G15 LICENSED PHARMACIST QUALIFICATION STANDARD The full performance level of this vacancy is GS-13. Physical Requirements: Considerable periods of sitting, standing, walking, reaching, and pushing or pulling of carts are inherent in the position. Bending, stooping, and lifting of supplies is necessary, mostly under 50 pounds
