The Central Specialty Care Clinic Registered Nurse (RN) Care Manager (CM) is a professional who provides high-quality, evidence-based care for Veteran patients who are cared for in the Specialty Care setting. The Registered Nurse Care Manager (RNCM) is a team member who provides comprehensive and coordinated nursing care to an assigned panel of patients.

Duties

1. Leads and organizes delivery of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning. 1.1. Uses sound clinical judgment in assessing, planning, implementing, documenting, and evaluating nursing care, based on the ANA Standards of Nursing Practice. 1.2. Guides staff in the application of critical thinking and process improvement to improve patient/resident care. 1.3. Anticipates and recognizes medical/surgical/psychiatric emergencies and initiates or leads others to implement appropriate and timely actions. 2. Uses advanced clinical knowledge/judgment to promote staff involvement in planning, decision-making, and evaluating outcomes. 3. Functions as an expert in clinical practice and/or areas related to the assigned roles and responsibilities. 4. Systematically evaluates current practice and formulates outcomes for groups of patient/residents and/or organizational processes within area of expertise. 5. Guides, develops, and supports staff from a leadership perspective. 6. Uses professional standards of care, scientific evidence, and practice to evaluate programs/service activities. 7. Demonstrates grade-appropriate and consistent critical thinking skills. 7.1. The nurse can recognize the patient/resident has a problem. 7.2. The nurse can manage the problem safely and effectively, recognizing his/her scope of practice. 7.3. The nurse has a relative sense of urgency. 7.4. The nurse takes the right action for the right reason (del Bueno). 8. Uses the Computerized Patient Record System (CPRS), pyxis and/or the Bar Coded Medication Administration (BCMA) software if applicable per policy. 9. Provides age-specific care for adult (18-64) and geriatric (65 and older) patients/residents/groups including the following elements: promotes independence; encourages self-care; involves family or designated caregiver with care planning, provides necessary safety measures and access to privacy; assists patient/resident to cope with changes affecting mobility, balance, cognition, psychosocial function, and physical/sensory changes. 10. Role-specific Competency Checklist is complete and is rated competent in all measured areas. 11. Acts as an expert consultant in clinical practice and/or in area(s) related to practice role responsibilities. 12. Assess non-scheduled patients using Ambulatory Care Policy/NSM and utilizes WAVE guidelines 13. Place consults and order lab/ diagnostic testing per Ambulatory Care Policy/NSM 14. Provide immunomodulating biologic or Kenalog IM injections 15. Practicing common nursing care procedures (e.g. suture/ clip removal, assisting with minor skin surgery, tuberculin skin tests, DUSA BLU-U photodynamic therapy, assisting with Mohs surgery, assisting with skin biopsy, assisting with application of liquid nitrogen, ultraviolet light treatments, etc.) 16. Teamleading for the flow of the Clinic 17. Answer view alerts/secure messages within 24-48 hours 18. Perform reminders as appropriate (influenza, audit-c, depression, PTSD, etc.) 19. Perform daily huddle with team Central Specialty Clinics RN Care Manager 20. Promote non-face to face visit support (MHV/secure messaging, telephone access/ appointments, VVC) 21. Care Manager to cover duties of Case Manager upon absence 21.1 Track and manage patient's paperwork 21.2 Use database resources for chronic care management (i.e. Dashboard, etc.) 21.3 Case management of the patient (e.g. follow-up with patient for continuity of care purposes/assessment, coordinating with external departments for care benefit of the patient) 22. Ensure daily clinic opening and closing duties have been completed (e.g. collect lidocaine syringes for dermatology clinic, EKG check, hand hygiene, Accu-chek controls, fridge temperature checks, EOC room checks, expired supplies, clean rooms, Safe Patient Handling equipment checks, clean and dirty utility room checks, specimens have been sent to the lab, rooms stocked with supplies, rooms locked upon clinic being closed for the day, verify no patients are remaining in the clinic) 23. For Allergy clinic, ensure patients are scheduled appropriately, medications are ordered, providing medication nebulizing treatments if necessary following Allergy Clinic Policy and/or provider orders. 25. For Pulmonary, Case Management of Pulmonary patients with COPD, chronic pulmonary diseases to ensure support of facility SAIL metrics and improvement of health of patient population. 26. For Nephrology, Case Management of Nephrology patients with level 3a, and 3B or higher Chronic Kidney disease, chronic kidney diseases to ensure improvement of health of patient population and support of facility SAIL metrics. and many more not listed above Work Schedule: 0800-1630 or depending on service needs. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Heavy lifting (45 lbs. and over); Moderate Carrying (15-44 pounds); Straight Pulling; Pushing; Reaching above Shoulder; Walking; Standing; Repeated bending; Ability for rapid mental and muscular coordination simultaneously; Near vision correctable at 13" to 16" to Jaeger 1 to 4; Hearing aid (permitted); Ability to distinguish basic colors; Ability to distinguish shades of colors; Depth perception; Both eyes required; Both legs required; Use of fingers and both hands required.
