This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws The Clinical Pharmacy Practitioner (CPP) functions at the highest level of clinical practice and top of licensure and serves as an advanced practice provider working independently, but collaboratively, with prescribing privileges as outlined in the individual's practice area-based scope of practice. The CPP provider delivers evidence-based comprehensive medication management (CMM) through direct patient care and as part of the interdisciplinary team providing Pharmacogenomic (PGx) management. The PGx CPP provider will incorporate a "personalized, proactive, patient-driven" approach to care that empowers and equips Veterans to take charge of their health and well-being and to live their life to the fullest in accordance with overall VA initiatives. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring, and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments that take into account the Veteran's PGx results. Further, deprescribing is employed where clinically indicated and informed by a Veteran's PGx results. The CPP provider serves to help achieve positive patient centric outcomes (e.g., establishing ad progressing towards individualized function-related goals, improving quality of life, minimizing theoretical risk of drug-related harm) through direct and indirect interactions with patients, providers, and interdisciplinary teams where applicable; performing physical assessments; implementing risk mitigation strategies and ordering laboratory (e.g., PGx panels, single gene tests, phenotype tests, therapeutic drug monitoring, etc.) and other tests t help maximize efficacy by initiating, modifying, and/or discontinuing medication both independently and in collaboration with other practitioners when needed. The CPP provider will follow patients in direct care settings to achieve therapeutic goals and make referrals as needed to optimize patient outcomes and treatment success. The CPP provider refers to the appropriate level of care for patients when needed and is responsible for providing and documenting care of assigned patients. The CPP provider has has knowledge of VA/DOD clinical practice guidelines, The Joint Commission (TJC) standards, Veterans Affairs (VA) performance measures, VA Pharmacy Benefits Management (PBM) initiatives and will integrate these standards into practice. The PGx CPP provider will participate as a subject matter expert on PGx-related committees at the local facility level (e.g., PGx Steering Committee) and as permitted at national and VISN levels. The CPP provider should assist with quality improvement projects related to PGx. Indirect patient care activities include teaching/mentoring trainees, participating in quality assurance and improvement projects, participating in PGx-based education programs, supporting research, staff education and consultation. The CPP provider will perform their duties within clinics or may work virtually using telehealth technologies when the team requirements do not necessitate a full time CPP provider be physically present. The CPP provider will be given appropriate resources and physical space (i.e., treatment room) for the direct care of their patients. Performs other duties assigned. Education (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) c. Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16 May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS 13 (a) Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. (b) Assignments. Candidates at this grade level are to be in one of the assignments listed below. For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time. Clinical Pharmacy Specialist. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPP plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Pharmacists assigned to this position must demonstrate the following KSAs: a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. b. Expert understanding of regulatory and quality standards for their program area. c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
