This position is located in the VA Medical Center's Primary Care and Mental Health Lines at the Coatesville VA Medical Center reporting to the Business Manager of Primary Care. The purpose of this position is to provide Patient Centered Management Module (PCMM) entry and technical support. As well as PCMM coordination and data evaluation and analysis.
Duties
Major duties include but are not limited to:
Collaborates and provides assistances and technical level support to the MC staff and leadership including but not limited to Clinical Managers, Providers, Nurses, Clerks, and the clinic staff
Responsible for maintaining the PCMM database throughout the hospital service area.
Makes assignment as well as removal of patients from the database when appropriate through ongoing PCMM data analysis.
Tracking and documentation of all administrative activities and requirements for the facility's Primary Care Management Module package.
Manages functions associated with the PCMM program for the facility. This includes attending required meetings etc. associated with the PCMM coordinator role and PCMM program.
The PCMM Coordinator is instrumental in strategic planning for Primary Care in addition to being a participant in strategic planning at the service level.
Performs work in direct support of an administrative area such as: management analysis or operating program analysis for the Primary Care Management Module package.
Extracts data and conducts data processing and analysis. Abstracts, edits, and/or codes patient records or other pertinent data. Data will be compiled and analyzed by others to provide comprehensive feedback to management.
Monitors service trends and makes recommendations on appropriate action or suggests alternatives for identified problems.
Ongoing critical thinking and analysis of workflow to identify potential problem areas in addition to making recommendations to correct these areas and to improve coordination of care.
Makes recommendations and suggestions to management that are vital in ensuring performance monitors/measures are being met and to ensure the MC adheres to local, VISN and national guidelines, memorandums and directives.
Work Schedule: Monday - Friday., 7:30am - 4:30pm

Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 01/22/2024.
Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-7 position you must have served 52 weeks at the GS-6. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment.
You may qualify based on your experience and/or education as described below:
Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-6 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to:
1) Identifying problem areas based on observation or discussion with clerks or health care officials/providers and initiates action to resolve problems.
2) Maintaining the Patient Centered Management Module database for all of primary care throughout the hospital service area.
3) Working with Team Leaders and Clinic Managers to formulate and conduct training and orientation for all staff, providers, and clerks involved with PCMM and clinic profile management.
4) Evaluating computer generated appointment types and determines appropriate assignments of appointment type on a timely basis.
5) Extracting data and conducts data processing and analysis including coding patient records or other pertinent data.
