The Senior Social Worker is the Coordinated Entry (CE) Specialist for Homeless Programs. This position enables VA homeless programs to fully collaborate with the community, including the Continuum of Care (CoC) and community partners, in the coordinated entry efforts in that community. The Senior Social Worker has experience that demonstrates possession of advanced practice skills and judgement in clinical services for homeless Veterans.

Duties

The Senior Social Worker is the VA liaison for coordinating outreach efforts with local community partners; including ensuring linkages to VA services and the local SPC for those Veterans who are experiencing literal street homelessness and are at high risk for suicide. Duties include the following, but are not limited to: Participate in CoC meetings and planning efforts which may include leadership roles within the CoC group. Provide direct linkage from community services to VHA homeless program services through regular and consistent outreach and communication with community providers. Participate in case conferencing process within the CoC's coordinated entry system to help inform and facilitate referrals to VA programs such as HUD VASH, SSVF, HCHV CRS, and GPD. Ensures efficient sharing of Veteran data and program information, as allowable under VA Privacy and Information Security policies and Directives. Provide support to the VA homeless program teams as a functional member of the team, to include participation in outreach activities and subject matter consultation on community involved interventions for homeless Veterans. Qualifications

Basic Requirements: The following are the basic requirements and qualifications for appointment as a Social Worker in the Veterans Health Administration (VHA). United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency Requirement - Per VA Handbook 5005, Part II, Chapter 3, Section A, Paragraph 3j: No person will be appointed under authority of 38 U.S.C., chapter 73 or 74, to serve in a direct patient-care capacity in VHA who is not proficient in written and spoken English. Education Requirement - You must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Note: Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree. Transcripts Required Licensure: Candidates must possess a full, current, and unrestricted license to independently practice as a Social Worker at the advanced practice level in a State, Territory, Commonwealth of the United States, or the District of Columbia. Copy of Licensure Required. Grade Determination: In addition to meeting the basic requirements as stated above, you must have the following experience to qualify for this GS-12 Senior Social Worker. Experience GS-12 Grade Level: The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Licensure/Certification. Individuals assigned as a Senior Social Worker must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. Physical Requirements: The work is sedentary and is usually accomplished while the employee is comfortably seated at a desk or table. Some walking and standing may occur in the course of a normal workday in connection with attendance of meetings and conferences, or while researching files. Items carried typically are light objects such ascases, notebooks, and data processing reports.
