The VA Greater Los Angeles Healthcare System (VAGLAHS) is seeking to hire a highly skilled &experienced Registered Nurse to serve as Wound & Ostomy Care Staff RN in Non-Institutional Care: Integrated Community Based Care. RN to serve as consultant for complex drainage, wound and/or ostomy problems. Will ensure that Veteran-centered care is provided & delivered in a quality, timely, accessible and respectful manner consistent with Veterans Health Administration mission, goals and objectives

Duties

VA offers a comprehensive total rewards package: VA Nurse Total Rewards Typical duties include but are not limited to: Responsible for planning, coordinating, providing and evaluating care for groups of patients in multiple level of care. Specialized RN who has the expertise and demonstrates advanced competencies in comprehensive skin assessment, providing staff education for the provision of assessing and prescribing topical wound and ostomy care treatment. Must have expertise to teach all inpatient staff to perform and document daily pressure ulcer risk assessment, daily skin assessment, Accurately staging any existing pressure ulcers with strong knowledge and practice on how to provide appropriate wound care treatment. Delivers safe care to assigned patients, while developing technical competencies. Safely and effectively uses patient care equipment. Correctly and safely performs nursing procedures. Recognizes abnormal signs and symptoms and/or changes in patients' condition and responds appropriately. Implements physician's/provider's orders in a timely and complete manner. Implements nursing interventions/orders in a timely and complete manner. Requests assistance from co-workers when needed; knows when to ask for help. Completes initial assessment of patient condition and response to treatment and initiates follow-up as needed. Includes the patient and family/significant other in developing the initial plan of care and establishing goals. Updates plan of care with assistance as needed. Assesses patient's learning needs, learning preference and readiness to learn. Develops and implements a plan to meet learning needs, instructing patients and families In basic health teaching and discharge planning. Documents the nursing process. Evaluates and documents patient response to interventions and progress towards attainment of goals and expected outcomes. Revise's plan of care according lo evaluations of patient's responses and outcomes. Demonstrates grade-appropriate and consistent critical thinking skills. The nurse can recognize the patient has a problem. The nurse can manage the problem safely and effectively, recognizing his/her scope of practice. The nurse has a relative sense of urgency. The nurse takes the right action for the right reason. Maintains professional boundaries with patients, families, and employees. Behavior as a role model is transparent and above reproach. Safeguard's patient privacy and maintains confidentiality of all electronic and printed patient information according to HIPAA guidelines, the Privacy Act, and as stated in the Medical Center Memo on ADP Security. Demonstrates compliance with infection control practices, such as hand washing and wearing of appropriate personal protective equipment. Demonstrates safe work practices. Complies with safety rules and regulations for safe job performance and fire protection. Practices safe body mechanics uses mechanical devices, and/or requests help when needed in moving, lifting, or turning patients to prevent injury to patient, self or others. Promptly reports all accidents to supervisor and notifies supervisor promptly of unsafe or unhealthy environmental situations/conditions. Corrects safety hazards promptly. Demonstrates positive, effective communication skills and professional behaviors that promote cooperation and teamwork with internal and external customers. Demonstrates effective written and verbal communication skills Demonstrates professional and cooperative behavior in all aspects of practice. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Bachelor of Science in Nursing Demonstrated ability to assess and prioritize multiple medical and surgical diagnoses Demonstrated excellence of physical assessment skills Demonstrated excellence in interpersonal and communication skills Demonstrated ability to be dependable, self-directed, and flexible including flexibility in assignment and time scheduling Evidence of self-direction or organizational skills. Evidence of effectively educating patients and patient families Proficient in the use of Microsoft Office programs or comparable to word processing, spreadsheet, and graphic software and the ability to learn new programs specific to the VA BLS Certification Advanced Cardiac Life Support (ACLS) certification, APIC certification is desired Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. For foreign education equivalency the only certifying body the VA recognizes is CGFNS and its' subsidiary, International Consultants of Delaware (ICD). Without certification of BSN-equivalency, the VA can only consider the degree at the associates level and pay will be determined accordingly. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Heavy lifting (35 lbs and over); Heavy carrying (35 lbs and over); Straight pulling and pushing; reaching above shoulder; use of fingers; both hands required; walking and standing up to 8 hours; repeated bending; Ability for rapid mental and muscular coordination simultaneously; hearing aid permitted. All registered nurses with less than 12-months of RN experience upon entry on duty (EOD) date, will complete a 12-month nurse residency program.
