This position is organizationally aligned under Social Work Service, Caregiver Support Program (CSP), at the Jesse Brown VA Medical Center (VAMC), Chicago, IL (537), a complexity level 1b facility. Jesse Brown VAMC consists of a 200-bed acute care facility and four community-based outpatient clinics (CBOCs). The medical center provides care to approximately 62,000 enrolled Veterans who reside in the City of Chicago and Cook County, Illinois, and in four counties in northwestern Indiana.

Duties

The Caregiver Support Program(CSP) Supervisory Social Worker, Program of Comprehensive Assistance for Family Caregivers provides services, education and resources to caregivers of Veterans. Overseeing service delivery and coordination of care activities for family caregivers providing personal care services to eligible Veterans regardless service period. CSP includes: Program of Comprehensive Assistance for Family Caregivers (PCAFC) and Program of General Caregiver Support Services (PGCSS). Major Duties: Provides clinical evidence-based services and interventions, program development, caregiver, Veteran and staff education on caregiver issues, community outreach, resource development, continuous quality improvement activities, evaluation/consultation, and supervision of staff. Performs the administrative and personnel management function relative to PCAFC staff supervised including personnel actions, clinical supervision, consultation, and negotiation with other departments. Develops, implements, and evaluates clinical guidelines and protocols to establish appropriate utilization of services. Assists CSP Facility Program Manager with strategic planning, continuous quality-improvement activities, and contributes to the program's resource and fiscal management. Provides supervisory coverage as needed for the CSP Facility Program Manager. Develops and facilitates PCAFC-program focused direction, orientation, coaching, in-service training, staff development, and continuing education programs for VA facility staff and the community. Recognizes gaps in workforce competencies and ensures supervised staff who require knowledge and skill development receive the training to necessary to execute their duties. The incumbent certifies staff completion of all required annual and ad hoc trainings through documentation and reporting. Develops a robust PCAFC that not only administers the required programing and services outlined within policy and regulation but also actively promotes national VHA resources on a consistent basis. Manages all processes, procedures, and practices associated with PCAFC eligibility and participation (i.e., enrollment, level of care, and discharge) to ensure application processing, tier adjustments, and program revocations follow local Standard Operating Procedures (SOPs) and align with national policy and governing regulations for timeliness and quality process and practice outcomes. Responsible for managing and reporting on program specific fund control points, labor mapping, workload, and clinic utilization within the assigned program area. Maintains a current network of referral resources to include peer support mentoring groups, caregiver skill building and self-care educational opportunities, mental health support, legal and financial services. Develops and implements a plan for outreach to Veterans and their caregivers who may benefit from the CSP program services. This includes outreach to current PCAFC participants to inform of available services. Serves as the major consultant for information on negotiating the complex processes caregivers, Veterans and families encounter when interacting with government and community agencies. Oversees the daily operations of the PCAFC and develops policies and procedures for program operation. Responsible for monitoring performance measures at the local, VISN, and Central Office levels, addressing VISN and VA Central Office action items and providing responses to congressional inquiries. Collects, analyzes, and reports program data to the Program Manager, facility leadership, VISN Caregiver Support Coordinator Lead and the National Caregiver Support Program, Care Management and Social Work Service, Office of Patient Care Services. Works collaboratively with internal staff, VISN and VACO leadership, and external stakeholders to expedite service delivery. Provides consultation and briefings to other staff and internal/external stakeholders on the special needs of caregivers. Serves as the VA HCS Point of Contact for all inquiries and outreach requests regarding eligible PCAFC caregivers. Stays abreast of current federal and state legislation directed at assisting caregivers to continue to provide caregiving in an environment that is safe for both themselves and the Veteran. Identifies unmet needs of the caregiver population, their families/significant others and caregivers. Works collaboratively with facility, VISN, and VA Central Office staff to develop and implement solutions. Qualifications

Basic Requirements: The following are the basic requirements and qualifications for appointment as a Social Worker in the Veterans Health Administration (VHA). English Language Proficiency Requirement - Per VA Handbook 5005, Part II, Chapter 3, Section A, Paragraph 3j: No person will be appointed under authority of 38 U.S.C., chapter 73 or 74, to serve in a direct patient-care capacity in VHA who is not proficient in written and spoken English. Education Requirement - You must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Note: Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree. Transcripts Required Licensure: Candidates must possess a full, current, and unrestricted license to independently practice as a Social Worker at the advanced practice level in a State, Territory, Commonwealth of the United States, or the District of Columbia. Copy of Licensure Required. In addition to meeting the basic requirements as stated above, you must have the following experience to qualify for this GS-12 Supervisory Social Worker. Experience GS-12 Grade Level: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as a Supervisory Social Worker must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (1) Ability to independently organize work, set priorities, and meet multiple deadlines. (2) Knowledge of supervisory duties which includes clinical supervision, consultation, negotiation with other departments and quality improvement. (3) Ability to ensure provision of clinical social work services. (4) Ability to provide staff training and development. (5) Skill in interpersonal relationships in dealing with employees, team leaders, and managers.
