The primary purpose of the position is to serve as a Community Care Coordinator. It is a foundational role within the continuum of patient care, working collaboratively with the patient care teams as well as a variety of specialty health care providers. This includes registered nurse care managers, primary care and/or specialty care providers, pharmacists, social workers, dieticians, behavioral health specialists, etc. to provide a robust interdisciplinary approach to care.
Duties
Major duties:
Provides administrative support to clinicians and other administrative and professional staff throughout the medical staff office for purposes of coordinating Non-VA medical care for veterans when inpatient and outpatient, and their VA provider has deemed such care necessary.
Administrative scheduling and appointment coordination for diagnostic testing, and consultative appointments for many clinical services.
Responsible for generating authorizations for pre-authorized medical care, while making eligibility determinations for the various programs in Community Care.
Works closely with utilization review to ensure proper authorizations are issued and appropriate correspondence is sent to vendors.
Obtains requested reports, x-rays, labs and any pertinent information required to complete the Community consult in CPRS.
Prioritizes requested procedure based on urgency, combat veteran status and compensation and pension exams.
Responsible for the coordination of all administrative information between the professional staff and the patient.
Provides advisory and technical assistance to veterans and staff in answering questions.
Coordinates the completion of Community Care consults assuring the corresponding report is linked to the appropriate consult, progress note title and patient.
Develops continuous monitors dealing with both clinical and administrative quality assessment and improvement activities.
Screen/receive phone calls in a courteous and timely manner, determine the nature of requests and provide the information desired using privacy rules and established processes.
Explains the VA's mandate to collect insurance information to veterans, their families, and other eligible patients.
Work Schedule: Full time, Monday - Friday, 8:00am - 4:30pm

Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS 5 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: Performing clerical/administrative duties in an office environment; answering and directing phone calls; preparing correspondence; maintaining files/records; and using a computer to perform a variety of support tasks; thorough working knowledge of clinic processes and services to function in and interact with various clinics throughout the hospital.

Note: A full year of work is considered to be 35-40 hours of work per week. Part-time experience will be credited on the basis of time actually spent in appropriate activities. Applicants wishing to receive credit for such experience must indicate clearly the nature of their duties and responsibilities in each position and the number of hours a week spent in such employment.
Physical Requirements: The work is primarily sedentary at a desk and workstation area. However, there may be some walking, standing, bending, and carrying of light items like papers, reports, files, or possibly moving computer equipment. May stand to greet visitors and escort people to meeting areas. No special physical demands are required to perform the work.
