VA Illiana Health Care System has a Clinical Pharmacist opening at their Danville VA Medical Center. Our clinical pharmacists ensure the safe and appropriate use of medications; and advocate rational drug therapy through evaluation of the appropriateness of drug therapy based on patient specific factors, individualization of drug therapy, providing medications and drug information.

Duties

Assignments involve a wide range of pharmacy services including those of a complex and highly specialized nature. This includes the proper verification, labeling, compounding, and dispensing of medications, the preparation of sterile I.V. admixtures and solutions, chemotherapy, and hyperalimentations. Render professional advice to physicians, consultants, and nursing personnel regarding medication dosing and dosage forms, routes of administration, frequency of administration, storage, and preparation. Communicate with providers and nurses as well as other pharmacists, technicians, and clerical staff. Consultant on pharmaceutical matters to various personnel in all aspects of the operation of pharmacy at this medical center. Perform Performance Improvement activities, including participation in meetings and educational programs, collecting and reporting data. Identify problems and offers solution proposals when assigned specific team leader functions. Provide on-the-job training to new employees, pharmacy students, and volunteers. Apply information gained at meetings by regular review of meeting agendas and minutes. Maintain knowledge of trends in the practice of pharmacy and controlled substance handling. Ensure pharmacy service follows federal laws, national and VISN guidance, local policies and any standards established by regulatory bodies (i.e., Joint Commission). Serve as a drug information resource by providing up-to-date drug information to prescribers, other health care professionals, patients, and caregivers. Provide guidance and functions as a role model to less skilled personnel in attaining increased proficiency. Verify the work produced for and by the pharmacy technicians as legally specified by licensure and as per policies established at this facility. Perform the work as assigned and to assist other positions as volume and workflow dictates in other work areas. Use judgment and initiative to ensure that the work of the pharmacy is completed daily. Participate in development of educational programs and provide in-services beneficial to pharmacy and its staff. Protects printed and electronic files containing sensitive information from unauthorized release or from loss, alteration, or unauthorized deletion under the provisions of the Privacy Act of 1974 and other applicable law and regulations. Follow applicable regulations and instructions regarding access to computerized files, release of access codes, etc., as set out in a computer agreement which the employee signs. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizen: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language: You must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. OR Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination(FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to the Basic Requirements above, applicants must meet the following grade requirements. GS-12 Clinical Pharmacist: Experience: 1 full year of experience equivalent to the next lower grade level, (GS-11) where you served in a developmental capacity after earning education cited above. Residency and fellowship training programs in a specialized area of clinical pharmacy practice may be substituted for creditable experience on a year-for year basis. The pharmacy residency program must be accredited by the American Society of Health System Pharmacists (ASHP). OR Education: Completion - or pending completion - of an ACPE-accredited Pharm.D. program Demonstrated KSAs: 1. Knowledge of professional pharmacy practice. 2. Ability to communicate orally and in writing to both patients and health care staff. 3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. 4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. References: VA HANDBOOK 5005/55, PART II, APPENDIX G15 LICENSED PHARMACIST QUALIFICATION STANDARD The full performance level of this vacancy is GS-12. Physical Requirements: Considerable periods of sitting, standing, walking, reaching, and pushing or pulling of carts are inherent in the position. Bending, stooping, and lifting of supplies is necessary, mostly under 50 pounds.
