Program Coordinators at the GS-12 grade level are administratively responsible for a clinical program providing services to Veterans in a major specialty area. The Program Coordinator may be the sole practitioner in this specialty at the facility and typically provides services in the program area.
Duties
The Program Coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice, and service delivery. The Program Coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. The Program Coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing, and implementing ideas for improvement and evaluating efficacy of improvement efforts.
The incumbent has subject matter expertise and experience with integrated healthcare services and interdisciplinary teamwork. The incumbent in this position will require a high degree of clinical knowledge and experience working with patients who are experiencing highly complex health or mental health problems, difficulty with adjustment to loss, elder abuse, self-neglect, dual diagnosis, domestic/Intimate Partner violence, substance abuse and other issues. The Social Worker has advanced practice skills to provide specialty care for complex, medically compromised veterans, as well as to provide consultation on complex cases and disposition issues to other social workers and to interdisciplinary staff. Incumbent will be able to provide insight and advisement to staff working throughout the medical center as well as assist in system changes to best meet the needs of the medical center and the patient.
The Social Work (SW) CC&ICM Program Coordinator will partner with the CC&ICM RN to implement the national CC&ICM framework which serves a patient population with highly complex health and mental health problems to ensure they receive the right level of care coordination support based on their prominent need. The SW CC&ICM Program Coordinator has both clinical and administrative responsibility to ensure effectiveness of services, application of latest VHA guidelines and modification of service patterns. The Primary Purpose of the Position is: to provide coordination and direct clinical evidence-based services including, assessments and interventions; program development; Veteran, family, caregiver, and staff education on Care Coordination (CC) and Integrated Case Management (ICM); resource development; community outreach; continuous quality improvement activities, and evaluation/consultation.
The SW CC&ICM Program Coordinator implements standardized tools designed to integrate care horizontally and vertically to include but not limited to, bridging medical care and mental health care, transitioning inpatient care and outpatient care, and integrating community care and VA care thereby creating a seamless continuum experience for the Veteran. Incumbent develops and implements a facility level framework for implementation of national standardized processes and tools that integrate communication, collaboration and coordination across services and programs to expand upon existing VHA program approaches and address concerns about care coordination service delivery gaps and service duplication.

Qualifications
Basic Requirements: The basic requirements for employment as a VHA social worker are prescribed by statute in 38 U.S.C. 7402(b)(9), as amended by section 205 of Public Law 106-419, enacted November 1, 2000. To qualify for appointment as a social worker in VHA, all applicants must meet the following:
Citizenship. Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part).
English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f).
Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates.
Experience/Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure.
Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs:
(a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
(b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
(c) Ability to supervise multidisciplinary staff assigned to the program.
(d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
(e) Ability to provide training, orientation, and guidance within clinical practice.
