This position supports creating authorizations for care in one or more of the programs included in the Home and Community Care (H&CBC) division of Geriatrics and Extended Care. These programs include Homemaker, Home health Aide (H/HHA), Community Adult Day Health Care (CADHC), Home Hospice, Home Respite, Veteran Directed-Home and Community Health Services (VD-HCBS).These programs require clinical and administrative quality oversight in order to meet VA Community Care standards.
Duties
Duties include, but are not limited to:
Collect, compile and track statistical information
Patients point of contact
Coordination of patients program
Support of program operations
Planning, reviewing, and reporting of data
Participate in performance improvement activities
Operate computer Software, such as, but not limited to VISTA, Word, Excel, Outlook
Work Schedule: Monday thru Friday 8:00am - 4:30pm

Qualifications

This position requires a typing requirement of 40 words per minute. Do you meet this requirement?
Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-06 position you must have served 52 weeks at the GS-05. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment.
You may qualify based on your experience as described below:
Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-05 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to:Communicating via phone, in person or writing to internal and external customers; Coordinating meetings and minutes; Utilizing Microsoft Suite and other computer programs for data and presentation development; Entering required data into the program database; Providing excellent customer service to all internal and external customers; Collecting, compiling and/or tracking of statistical information; Establishing protocols for incoming data; Preparing financial reports.
Preferred Experience:
Experience providing daily support as an office assistant or clerk for Community care processes. Responsible for creating authorizations in HSRM with a VA community care background and working with community agencies for placements.

Physical Requirements: The work is partly sedentary, using the computer for parts of the day. Walking to other parts of the facility may be necessary. Some bending and standing is required.
