The incumbent serves as the COMPACT ACT Program Coordinator for COMPACT ACT Program at Mountain Home. This position assumes the administrative/clinical responsibility for the management of the program and is directly responsible for the success of all direct patient care and support services provided, and is administratively responsible to ensure program compliance with national, local, The Joint Commission, and Commission on Accreditation of Rehabilitation Facility guidelines and directives.

The COMPACT ACT Program Coordinator the provides outreach, crisis intervention and case management services to Veterans at high risk for suicide. The incumbent provides services at an advanced practice level and provides clinical intervention through groups, individual/family crisis services and case management to eligible Veterans and their family members to meet biological, psychological, social, and housing needs.
Duties include but are not limited to:
Utilizes program coordination skills to develop and monitor administrative and clinical plans for Veterans receiving COMPACT Act services.
Oversees daily operations of the program and provides reports and metrics to facility, VISN and VACO as requested.
Captures and analyzes local data trends to inform program development and improvement.
Collaborates with local facility revenue and finance departments.
Advocates for Veterans, ensuring they do not receive bills for COMPACT related care.
Provides education and training on COMPACT Act to internal and external stakeholders as the facility subject matter expert.
Reviews and has knowledge of and develops policies, standard operating procedures, practice guidelines for the program.
Coordinates between health care providers and care managers in direct care and community care.
Collaborates with inter-professional teams within the VA healthcare system and community to navigate complex care coordination.
Consults and negotiates across program areas to ensure appropriate resources are secure in a clinically relevant and timely manner.
Provides clinical social work assessment, intervention, treatment, and follow-up, using a social determinant of health framework.
Oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use.
May be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds.

Qualifications

Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education - Candidates must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
Licensure - Social workers at the GS-12 must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
GS-12 Level: Social Worker (Program Coordinators) must have at least one year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Must have specialized knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Knowledge of medical and mental health diagnoses, disabilities, and treatment procedures to formulate a treatment plan. Knowledge in outreach, crisis intervention and case management services to Veterans at high risk for suicide. Knowledge of clinical intervention through groups, individual/family crisis services and case management, and are able to demonstrate the following knowledge, skills and abilities:
Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
Ability to supervise multidisciplinary staff assigned to the program.
Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
Ability to provide training, orientation, and guidance within clinical practice.
