This position serves as the Program Director of the Home Based Primary Care (HBPC) program, and oversees the daily operations of that program as well as generating and analyzing both VISN and National reports related to HBPC. HBPC is intended to serve as a clinically appropriate, cost effective and efficient alterative to lengthy hospital and institutional stays. It enables chronically and terminally ill Veterans to receive comprehensive health and supportive services in their homes.
Duties
The Home Based Primary Care (HBPC) Program Director/Social Worker (SW) Program Coordinator is Social Worker; he/she is accountable for the management, supervision and evaluation of care for patients enrolled in the HBPC program. He/she facilitates an atmosphere of interactive management and development of collegial relationships among interdisciplinary team members, serving as a link between Geriatrics and Extended Care Service and all healthcare disciplines included on the HBPC teams. Duties include, but are not limited to:
The HBPC Program Director/SW Program Coordinator is responsible for directing the clinical and administrative services offered by the program to ensure that the program is in compliance with local and VHA standards and policies as well as accreditation standards for home care organizations.
He/she is responsible for the development and continued effective functioning of the interdisciplinary health care team.
He/she recognizes and utilizes the skills, knowledge and contributions of each team member.
He/she collaborates with the HBPC Medical Director for daily program management and collaborates with other HBPC program Directors, Veteran Integrated Service Network (VISN) leadership and VA Central Office staff on a regular and ongoing basis.
He/she is responsible for the interpretation and communication of VA policies to the HBPC team and the VHA facility.
He/she is responsible for the evaluation of the effectiveness of the program and assuring effective/efficient resources are maintained.
Responsible for the regular review and reporting of quality metrics. Instrumental in initiating and implementing quality improvement projects.
He/she assures the program is able to optimize cost effectiveness (budget) and patient outcomes.
All other duties as assigned.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.
Grade Determinations: Social Worker (Program Coordinator), GS-13
(1) Experience/Education. One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
(2) Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
(3) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs.
Skill in coordinating and implementing one or more specialty program(s), across the facility.
Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive.
Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
Ability to collaborate with internal and external partners to further program goals and enhance patient centered care.
Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action.
Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget.
Knowledge of organizational structure and impact on program operations.

The full performance level of this vacancy is GS13.
Driver's License: The incumbent will need to drive a car and have a valid state license. The position often places the incumbent in a position of stress. Frequent/daily travel is required and overnight travel may be required when multi-day stays are necessary.
Physical Requirements: All applicants must be physically able to satisfactorily perform the duties of the position. A pre-placement physical examination shall be completed prior to appointment.
