The VA Portland Health Care System is a Complexity Level 1a Facility and provides extensive primary, secondary, tertiary, and quaternary care. The Housing and Urban Development-Veterans Affairs Supportive Housing (HUD-VASH) is an intensive case management program for Veterans who struggle with housing and employment instability due to complex psychiatric and medical diagnoses. This position is responsible for the development and monitoring of contract providers serving the participants.
Duties
The Social Worker (Program Coordinator) serves as a Clinical Contract Specialist (CCS). The Clinical Contract Specialist is the key point of contact between VA Portland Health Care System and a community agency that has been awarded VA HUD-VASH contract dollars to provide HUD-VASH housing and case management services. The Clinical Contract Specialist will obtain Contracting Officer Representative (COR) I and II certification and is responsible for performing all duties expected of this title.
Major Duties include, but are not limited to:
Responsible for monitoring the contractor's ability to fulfill the terms of the contract.
Identifies and addresses ethical issues that impact patients and staff, initiating and participating in ethical consultations with contracted providers.
Reviews documentation of the contracted agency and conducts site inspections, ensuring that all regulations are met, and any deficiencies documented with timelines for corrective action to be completed.
Reviews resumes of contracted staff to ensure meets the minimum clinical requirements and is the POC for the contracted agency when onboarding new contract staff and assists in submitting all paperwork needed for background checks.
Maintains database and ensures accurate information and content created by the Contractor in the electronic health record is recorded within required timelines in order to document, evaluate, and enhance the quality of service provided.
Develops and implements information systems to track service activities including visits, referrals, inpatient days, patient demographics, equipment utilization and patient satisfaction.
Performs work related to the review, monitoring, analysis, planning and assessment of the HUD-VASH program and its contracted provider(s).
Shares expertise with internal and/or external customers to the organization by leading or actively participating in interdisciplinary groups such as committees, taskforces, projects, formal teaching, publishing, or professional activities.
Reviews contract and is independently responsible for monitoring compliance of the contracted provider through established performance measures.
The Clinical Contract Specialist will obtain COR I and II certification and is responsible for performing all duties expected of this title.
Collaborates with Supervisory Social Worker(s), Program Manager(s), Program Coordinator(s), and VISN Contracting Officer to develop a statement of work for initial and ongoing solicitation of the contract.
Coordinates with the program Administrative Officer, Finance representative, and designated staff member(s) within the CRS program to ensure for appropriate and timely fund obligation and certification of payment for services rendered.
Responsible for annual contracted program performance report and provides provider and Contracting Officer with findings and corrective actions, as indicated; report and corrective actions outside of the annual performance period, if required.
Maintains accurate and up-to-date administrative records for each contracted program.
Monitors, tracks, and reports entries of contracted provider(s), monitoring utilization of vouchers , including intake, referral, vouchering, lease-up, graduation, and/or termination/voucher loss.
Assists leadership with CARF and Joint Commission preparation including writing Standard Operation Procedures (SOPs) and other service level guidance documents.
Provides consultation and supervision to social work trainees and unlicensed staff.
Work Schedule: Monday through Friday, 8 a.m. to 430 p.m.

EDRP Authorized: This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Qualifications
BASIC REQUIREMENTS
The basic requirements for employment as a VHA social worker are prescribed by statute in 38 U.S.C. § 7402(b)(9), as amended by section 205 of Public Law 106-419, enacted November 1, 2000. To qualify for appointment as a social worker in VHA, all applicants must meet the following:
Citizenship. Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part).
Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
Grade Determination for Social Worker (Program Coordinator) GS-12: In addition to the basic requirements stated above, the following qualification criteria must be met for each grade. The candidate's qualifications must clearly demonstrate the level of competence required for the grade.
Experience and Education : One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
Ability to supervise multidisciplinary staff assigned to the program.
Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
Ability to provide training, orientation, and guidance within clinical practice.
Preferred Experience: Contracting Officer Representative (COR)experience. Experience in homeless programs, experience with data systems, experience with oversight of professional staff such as team lead or supervisory roles.
