This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
Duties
The Caregiver Support Coordinator assists with the implementation of the Caregiver Support Program specific to VA North Texas Healthcare System and ensures compliance and collaboration with local facility, VISN and VA Central Office Caregiver Support Program initiatives. The incumbent is responsible for the planning, development, implementation, monitoring, and evaluating of program operations. Goals and requirements of the program are accomplished through a collaborative team effort among the Caregiver Support Coordinators.
Duties include but are not limited to the following:
Utilizes data to evaluate and enhance the quality of service provided to caregivers, families, and Veterans.
Develops and implements information systems to track service activities including visits, referrals, caregiver demographics, equipment utilization and caregiver and Veteran satisfaction.
Monitors and assist Veterans and Caregivers through the 10-10CG application process.
Develops, implements, and evaluates clinical guidelines and protocols to establish appropriate utilization of services.
Participates in direct caregiver and Veteran contacts to identify and assess caregiver stress/burden and the development and application of directed clinical treatment interventions.
Creates educational tools, develops programs, and implements training focused on specific caregiver needs/issues. Provides education and in-service training to VANTHCS employees, Veterans, families, and caregivers. Provides ongoing consultation and support to colleagues regarding caregiver support.
Develops and facilitates caregiver support groups and educational programs (face-to-face or telephone) as needed.
Engages in assessments, groups and coaching with Veterans and Caregivers through in-person visits, home visits, VVC and telephone modalities.
Conducts caregiver and family clinical interventions to facilitate the identification of caregiver stress, psychiatric illness, emotional distress, and social needs as treatment issues. Professional judgment, including the knowledge of normal and abnormal behavior, is an inherent competency applied in daily interactions with caregivers and Veterans.
Provides referral to VA medical and mental health treatment and to other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs.

Develops a library of caregiver support information material for distribution through the various inpatient/outpatient and specialty clinic settings.
Serves as a liaison between the VA and community agencies. Participates in caregiver support activities in the local community and state to promote and recognize the role of the caregiver.
Participates in outreach events as required. Community outreach may require a change in tour as needed to cover evening and weekend events as appropriate.
CSCs collaborate to coordinate event and activities for the annual National Family Caregivers Month every November.
Assists the Program Manager, or designee, with the development of policies and procedures for program operation, as needed.
Prepares reports for facility, VISN, and national use, as needed.
Ensures compliance with local and system wide programs, policy manuals and directives.
Identifies problem areas/issues that should be brought to leadership's attention.
Problem solves with Caregiver Support Program team to develop a plan to address issues.
CSCs collaborate in data collection for the purposes of monitoring the budget needs for the program and submitting reports, as requested by the Program Manager, or designee.
CSC collaborates with Veteran, Caregiver, and his or her appropriate treatment teams to coordinate rehabilitation and treatment goals for the Veteran to work towards independence while in the Caregiver Support Program.
This position will require travel in the community. Must be able to obtain and maintain a Federal Driver's License to operate a government vehicle.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: You must be a US Citizen.
English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Grade Determinations:
Experience: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.

Knowledge, Skills, Abilities KSA's: In addition to the experience above, candidates must demonstrate all of the following KSAs:
(a) Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.
(b) Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds.
(c) Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan.
(d) Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals.
(e) Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.
Assignments: Program coordinators at the GS 12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts.
