This RITS (Rehabilitation Intervention Therapy Service) and Community Care Coordinator is a Social Worker located at the Richard L. Roudebush VA Medical Center. Primary responsibilities include community collaboration, education, and referral management of all acute rehabilitation referrals. The Social Worker will also be responsible for assessment and discharge planning of all Veterans admitted onto the RITS unit at Veterans Health Indiana.

The incumbent is based in the Medical Center, although a significant amount of work occurs in the community. The RITS and Community Care Social Work Coordinator will meet face to face with Veterans who may be in other community facilities, to expedite the screening and admission process for the RITS unit at Veterans Health Indiana.
Must be a skilled communicator capable of developing effective partnerships with local, state, and federal agencies as well as non-governmental organizations.

The incumbent will function in support of Veterans, family members, and Caregivers throughout the medical center and community settings.
The incumbent will regularly provide education and support to community hospitals to provide continuing education surrounding VHI acute rehab unit and VHA benefits.
Practices at the advanced level and has specialized knowledge of practice that can be differentiated from the independent practice social worker by the ability to expand clinical knowledge in the profession, administer a facility program, provide consultation and guidance to staff, role model effective social work practice skills, teach or provide orientation to less experienced social workers, develop innovations in practice interventions, and provide clinical supervision for social work licensure or certification.

Assists in planning, establishing, and implementing policies and procedures; developing individual and/or group goals and objectives as well as monitoring, operating, evaluating, coordinating and overseeing CARF Social Work accreditation needs.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education Requirement - You must have a master's degree in Social Work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Note: A doctoral degree in Social Work may NOT be substituted for the master's degree in Social Work.
Licensure - You must be licensed or certified by a state to independently practice social work at the advanced practice level, and must be able to provide supervision for licensure. Indiana requires Licensed Clinical Social Worker-LCSW.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Experience: Must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. In addition to the experience above, the candidate must demonstrate the following knowledge, skills, and abilities:
Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management.
Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice.
Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes.
Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills.
Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.

The full performance level of this vacancy is GS12. The actual grade at which an applicant may be selected for this vacancy is at the GS12.
Physical Requirements: The physical demands of the position involve standard office requirements such as using the telephone, retrieving files, writing reports, data entry and maintenance of computer databases, etc. Work is typically performed sitting at a desk and using a computer. The position requires periods of walking, standing, sitting and bending.
However much of the work also occurs in the community/Veteran's homes, so the environment can change accordingly. Since the position has a large community-based focus, the position requires travel and driving a government owned vehicle, (GOV) and thus requires a valid driver's license and for the incumbent to remain in good standing with all trainings and regulations with regards to use of a Government Owned Vehicle.
