The Part-Time weekend Outpatient Clinical Pharmacist will be assigned to staffing or discharge duties within the Outpatient Pharmacy. The Clinical Pharmacist will staff weekends and may be scheduled for other days as needed by the service to manage workload and patient care efficiently. May be scheduled on Holidays for additional staff support.

Duties

Major Duties and Responsibilities Clinical Functions: 1. Provides drug information and appropriate recommendations regarding treatment alternatives. Offers interventions to maximize patient care outcomes, considering choice of therapy, safety, efficacy, and cost. 2. Evaluates patient charts for medication history, compliance, reconciliation and appropriateness of pharmacologic therapy. Communicates and documents all therapeutic related recommendations pertaining to patient care as necessary. 3. Identifies and documents any drug-related problems such as allergic reactions, toxicities, adverse reactions, improper dose regimen, noncompliance, etc. Medication Dispensing: 1. All medication orders shall be entered into the electronic patient data bank. 2. Medication profiles shall be reviewed and monitored for compliance or potential abuse; problems encountered shall be resolved. 3. The pharmacist will select, compound, dispense and fill a range of drugs including extemporaneous preparations, special formulations, ophthalmics (subconjunctivals and intravitreal injections), narcotics, investigational drugs, oncolytic agents, etc., as appropriate in both outpatient and inpatient areas. Adequate quality assurance procedures shall be followed as applicable. 4. Maintain adequate drug stock, inspect drug storage areas, supervise prepacking and the operation of automated dispensing equipment. Education: 1. Participates in the training and in-servicing of pharmacists, pharmacy technicians, pharmacy students, pharmacy residents and other staff as assigned. 2. Attends and actively participates in pharmacy related training, in-services, staff development, and relevant conferences. 3. Responsible for keeping abreast of changes in the Pharmacy Benefits Management (PBM), CPPO, and PBM Formulary services (e.g., formulary additions and deletions, relevant memoranda, and policies and procedures), as well as general knowledge of VA, Drug Enforcement Administration, US Food and Drug Administration, Office of Inspector General and The Joint Commission regulations. 4. Actively collaborates within Pharmacy Service and with other health care providers in the design and implementation of drug research protocols, collection of data, and in the analysis and interpretation of results as appropriate. Administrative: 1. Adheres to and ensures the pharmacy service is compliant with all regulatory and accrediting body requirements, includes pharmacy related policies and procedures. 2. Completes administrative projects, reports, assignments in a timely manner. 3. Participates in appropriate committees to contribute to safety improvements and initiatives as requested and appropriate. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. OR, Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination(FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). EXPERIENCE OR EDUCATION GS-12 LEVEL: In addition to the basic requirements, candidates must meet one of the following: One (1) year of directly related experience equivalent to the next lower grade level, OR, Completion of an ACPE-accredited Pharm.D. program. GS-12 Assignment: A pharmacist in this assignment handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations. These include, but are not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution. AND, Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs): 1. Knowledge of professional pharmacy practice. 2. Ability to communicate orally and in writing to both patients and health care staff. 3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. 4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
