This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU The Social Worker program Coordinator serves as the COMPACT Act Coordinator and has responsibility as the primary navigator for Veterans received care under COMPACT Act. This included coordinating transitions in care for Veterans received acute suicide care under COMPACT Act between varying venues of care, types of care (medical and mental health) and location of care (direct care and community care) in collaboration with assigned care coordinators at each step of the Veteran journey. The Social Worker Program Coordinator will be instrumental in providing connection and referral to assist with proper evaluation for eligibility in other programs and is the facility subject matter expert on the COMPACT Act. The Social Worker Program Coordinator is responsible for the implementation, coordination and monitoring of the COMPACT Act at the facility. Utilize program coordination skills to develop and monitor administrative and clinical plans for Veterans receiving COMPACT Act services. Oversees daily operations of the program and provides reports and metrics to facility, VISN and VACO as requested. Captures and analyzes local data trends to inform program development and improvement. Collaborate with local facility revenue and finance departments. Advocate for Veterans, ensuring they do not receive bills for COMPACT related care. Provide education on training and COMPACT Act to internal and external stakeholders as the facility subject matter expert. Reviews and develops policies, standard operating procedures, practice guidelines for the program. Coordination between health care providers and care managers in direct care and community care. Collaborate with interprofessional teams within the VA healthcare system and community to navigate complex care coordination. Consultation and negotiation across program areas to ensure appropriate resources are secure in clinically relevant and timely manner. Coordinate with local Patient Advocate Tracking System - Replacement (PATS-R) COMPACT clinical review teams Coordinates with local facility Office of Integrated Veteran Care (IVC) Care Coordinators. Provides training, education, orientation, and guidance on clinical program area. Provides clinical social work assessment, intervention, treatment and follow-up, using a social determinant of health framework. Assists with navigation between varying levels of care and conducts comprehensive transition planning. Utilize designated COMPACT Act templates and coding requirements when documenting in the medical record. Complete the Suicide Prevention Safety Plans while still in the non-VA inpatient facility. Facilitate appropriate hand off with VA Suicide Prevention Program staff to assure continuity of care when needed. Ensure Veteran's demographics and contact information are up to date and accurate in CPRS. May be responsible for a tour of duty that includes early morning, evenings and/or weekends and participation in the On-Call rotation schedule to accommodate Veteran care need during weekends, holidays, evenings, and nights. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area. Participate in the ongoing Suicide Prevention outreach efforts as needed. Qualifications

BASIC REQUIREMENTS. The basic requirements for employment as a VHA social worker are prescribed by statute in 38 U.S.C. § 7402(b)(9), as amended by section 205 of Public Law 106-419, enacted November 1, 2000. To qualify for appointment as a social worker in VHA, all applicants must meet the following: Citizenship. Be a citizen of the United States. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditationto verify that the social work degree meets the accreditation standards for a masters of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grade Determinations: Social Worker (Program Coordinator), GS-12 (1) Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. DEMONSTRATED knowledge, Skills and Abilities at the GS-11 Social Work Level are defined as: (a) Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services. (b) Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. (c) Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan. (d) Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. (e) Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. (2) Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. (3) GS-12 Program Coordinator demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice.
