The Maternity Care Coordinator (MCC) is a licensed social worker who is responsible for identifying, facilitating, and coordinating patient-centered care and services. The MCC has a crucial role in assisting women Veterans to efficiently obtain prenatal and postpartum care and specific services, with an emphasis on care coordination with the Veteran's Patient Aligned Care Team (PACT) and Community Obstetrician/Gynecologist (OB/GYN), for preventative health maintenance and reintegration.

Duties

FUNCTIONS OR SCOPE OF ASSIGNED DUTIES: a. Maternity Care Coordinator Responsibilities (90%): Duties include directing, administering, and managing all aspects of the program, including comprehensive planning, developing, and implementing programs, policies, and procedures; determining program needs; coordinating the type, quality and quantity of services provided; setting priorities; and establishing short and long-range program goals, initiatives, and direction. Demonstrates leadership and mastery in delivering and improving efficient, comprehensive, and continuous patient-driven holistic care through collaborative strategies with others that includes creative approaches to management of complex client care. Documents outcomes at the program or service level. Program is defined as a substantial coordinated group of activities impacting patients and/or employees in compliance with VHA Handbook 1330.03 Maternity Health Care and Coordination. Leads and organizes delivery of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning. Applies advanced clinical knowledge/judgment to promote staff involvement in planning, decision making, and evaluating outcomes. Functions as an expert in clinical practice and/or in areas related to the assigned roles and responsibilities. Systematically evaluates current practice and formulates outcomes for groups of patients and/or organizational processes within area of expertise. Uses professional standards of care, scientific evidence, and practice to evaluate programs and/or service activities and seeks opportunities to improve service delivery and reduce adverse outcomes through systems redesign processes. Monitors provision of services to be delivered in a manner that meets VA guidelines and standards to include access to care with on demand coordinator availability via telehealth modalities during regular duty hours. Coordinates and monitors gender-specific care outcomes (i.e., maternity care, breast care/mammography, gynecological coordination, etc.) of patients throughout all aspects of the medical center, including Community Based care providers, as well as community vendors and/or agencies. Provides education to patients and staff on a variety of topics, to include but not limited to prenatal education, maternity/newborn coverage and insurance options, importance of routine medical screenings, and authorized VA and community based resources. Facilitates care management utilizing community and VA resources such as mental health services/support, post-partum care, specialty lactation consultants, appropriate referrals (i.e., Pharmacy for medication management), and support groups (i.e., new-mothers, perinatal loss, multiple gestation, breastfeeding groups). Screens for mental health conditions and Intimate Partner Violence and facilitates specialty referrals when indicated. Ensures regular contact with women during the perinatal and post-partum periods, based on the needs of the patient, until their care is transitioned to PACT. Assists Veterans who have presented to a VA or non-VA emergency department for gender-specific emergencies (i.e., miscarriages) and facilitate appropriate follow-up care such as lab, ultrasound, emotional support, and/or other services as needed in coordination with the Veteran's PACT. Documents patient care interactions in the electronic medical record accurately, succinctly, and timely using appropriate notes, templates, clinical reminders, and electronic alerts (flag) for pregnancy/lactation. Participates in various committees, workgroups, and conference calls; represents the TVHS Maternity Care Coordination Program on all appropriate committees. Develops and submits reports to TVHS, VISN, VHA leadership and when necessary, develops and provides corrective action plans for areas identified as deficient in meeting established benchmarks. b. Community Engagement and Performance Improvement duties (10%): Develops and delivers education and outreach services that encompass underserved veteran populations in the facility's geographical catchment area and may include regional or state events. Utilizes multimedia campaign materials (e.g., printed articles, brochures, videos) to educate and engage target audiences. Qualifications

Basic Requirements: a. Citizenship. Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part). b. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. c. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation). Loss of Licensure or Certification. Once licensed or certified, social workers must maintain a full, valid, and unrestricted independent license or certification to remain qualified for employment. Loss of licensure or certification will result in removal from the GS-0185 social worker series and may result in termination of employment. d. English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f). GRADE DETERMINATIONS. Social Worker (Program Coordinator), GS-12 Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS-12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts Meets the qualification standard for the GS12 Social Worker as defined in See VA Handbook 5005, Part II Appendix G39, Social Worker Qualification Standard GS-185 Veterans Health Administration. Note: If the incumbent is LCSW or equivalent, must be able to provide clinical supervision for Social Workers seeking the licensure. Must be verified by the licensing board of the state where the license was awarded Reference: VA HANDBOOK 5005/50, Part II, APPENDIX G39. SOCIAL WORKER QUALIFICATION STANDARD dated FEBRUARY 29, 2012 Physical Requirements: Work is divided between office and community settings, including prisons or jails, courts, and other locations where justice involved Veterans may be found. May require prolonged standing, moderate walking, and manual dexterity. Requires carrying light items, such as books and files.
