The incumbent serves as a Social Worker Program Manager assigned to the MidSouth Healthcare Network, VISN 9 within the Rehabilitation and Extended Care (REC) Integrated Clinical Community (ICC) as the H&CBS Program Manager. The H&CBS Social Worker Program Manager reports to the REC ICC Lead/Program Manager.

Duties

The VISN 9 H&CBS programs encompasses both non-institutional care programs and community institutional care programs. Non-institutional programs include, but are not limited to, Home Health Aide/Homemaker, Respite, Bowel and Bladder, Contract Adult Day Healthcare, Medical Foster Home, Veteran-Directed Care, Skilled Home Health Care and Rehabilitation Specialty Services. Institutional programs include Community Nursing Home (CNH) and State Veterans Home (SVH) programs. VISN 9 is comprised of five health care systems distributed over multiple states in the Southeastern United States serving more than 381,000 enrolled Veterans. The five health care systems are of varying size and complexity: - Complexity level 1a: Tennessee Valley Healthcare System - Complexity level 1a: Memphis VA Healthcare System - Complexity level 1b: Lexington VA Healthcare System - Complexity level 1c: Louisville VA Healthcare System - Complexity level 1b: Mountain Home VA Healthcare System The five health care systems include a total of 53 community-based Veteran clinics and three community living centers The incumbent serves as a member of the REC ICC Leadership team and must be able to operate with autonomy to ensure facility and programmatic guidance is provided timely to meet the needs of Veterans requiring complex care and support. The incumbent has responsibility for work assignments, budgeting including cost monitoring and analysis, staffing and contractual guidance, and oversight. The incumbent performs a wide range of services, providing leadership and technical assistance for H&CBS programs and provides clear and consistent communication and collaboration in the VISN and five Medical Centers through development and implementation of programs, policies, staffing, resource allocation, education, patient care and procedures to meet goals, policy, and accreditation requirements. In addition, the incumbent uses sound judgement in the interpretation of trends and patterns; the application of regulation, policy, and guidelines; provides authorization guidance and clinical reviews; coordinates and designs information systems which support the utilization of patient data entry, as well as for collecting, aggregating, and analyzing data related to various types of 2 health care issues and performance management; facilitate systems redesign and quality improvement projects to optimize VISN 9 practices and lead both formal/chartered and informal work groups to assist with sharing and standardizing best practices across VISN 9; and provides recommendations for program enhancements. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a Master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. English Language Proficiency: Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j, this part. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-14 Social Worker Program Manager: 1)Experience and Education. One year of experience equivalent to the GS- 13 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Individual may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. (2) Licensure/Certification. Individuals assigned as social worker program manager must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all the following KSAs: (a) Skill in large scale program coordination and administration across multiple sites, which includes consultation, negotiation, and monitoring. (b) Ability to translate management goals and objectives into well-coordinated and controlled work operations. (c) Ability to analyze organizational and operational problems and to develop and implement solutions that result in sound operation of the program. (d) Ability to collaborate with internal and external partners to further program goals and enhance patient centered care. (e) Ability to provide the full range of supervisory duties which would include responsibility for assignment of work to be performed; performance evaluations; selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action. (f) Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget. (g) Skill in developing policies, procedures, and/or practice guidelines for the program and in participating in national program development. (h) Ability to collaborate with internal VHA partners, locally and on a national level, as well as external community partners to further program goals and enhance patient centered care. References: VA Handbook 5005/120, Part II, Appendix G39. The full performance level of this vacancy is GS-14. Physical Requirements: The work is primarily sedentary administrative in nature. May occasionally require walking, standing, bending, carrying of light objects such as files, books and papers. Much of the work occurs within a typical office setting, either individually or in meeting with others, or in meeting rooms.
