The HCHV Case Manager, is a professional social worker, assigned to Eastern Kansas Health Care System's (EKHCS) Healthcare for Homeless Veterans Programs. The responsibility of this position is to identify and engage homeless Veterans and provide direct services and/or develop and connect homeless Veterans to services and resources that will enable them to secure safe, affordable, quality permanent housing.

The HCHV Case Manager provides outreach, assessments, referrals and case management services to the homeless Veterans identified in the community. Services are also provided to homeless veterans receiving treatment in contract residential treatment and supportive housing programs. Referrals come from various shelters, soup kitchens, drop-in centers, social service agencies and the medical center. The HCHV Case Manager completes program specific forms, including Homeless Operations Management System (HOMES) forms, on Homeless Veterans receiving outreach services. Efforts are directed toward using a therapeutic relationship to assist the individual/significant other gain more control over their lives and resolve issues which have lead to homelessness (i.e. substance dependence and/or mental illness).
In addition to direct services, the HCHV Case Manager provides program oversight, monitoring and administration by developing working relationships and agreements with other organizations and directly coordinates with HCHV contract provider program referrals, program admissions and is responsible for the evaluation of services provided by HCHV contract providers. The HCHV Case Manager initiates, participates and oversee the completion of annual inspections to contract HCHV residential treatment providers to assure VA regulations are met by community providers.
The HCHV Case Manager performs duties outlined in Social Work Professional Practice, VHA Handbook 1110.02 (6, 3 a-1).
Duties/Responsibilities include, but are not limited too:
Performs outreach and facilitates eligibility determination by securing military discharge papers if necessary and facilitating enrollment for care with VA staff and determining eligibility in accordance with the HUD HEARTH Act definition of homelessness.
Conducts psychosocial assessments; develops treatment plans in collaboration with the Veteran/family and the interdisciplinary treatment team. Possesses the knowledge and skills to independently implement treatment modalities and provide counseling or psychotherapy for individuals, families and groups.
Possesses the skills to perform psychosocial interventions and measure outcome results.
Coordinates community based services, including information and referral for additional services from other VA programs, other government programs and community agency programs.
Refers veterans to appropriate VA and other government/community services-including local shelters, food banks, employment services, veteran's benefits, and social security applications.
Assists veterans who encounter problems obtaining services from the VA and other service delivery agencies through advocacy, linking, and/or by assisting with problem solving/social interaction skills.
Identifies high risk patients and provides case management services.
Provides crisis intervention services, seeking to address the cause as well as presenting complaint, engaging family and other support systems as appropriate.
Has knowledge and experience in the use of medical and mental health diagnoses, disabilities and treatment procedures, including acute, chronic and traumatic illnesses/injuries, common medications and their effects, side effects, and medical terminology .
Establishes and maintains effective therapeutic relationships with Veterans and their families.
Works independently with patients and families who are experiencing a variety of psychiatric, medical and social problems, utilizing individual, group and family counseling and therapy skills.
Assists veterans in maintaining appointments at VA medical centers and other community agencies for medical, psychiatric, substance abuse, vocational and other services consistent with veteran's goals associated with healthful independent living.
Understands disparate values, and reaches veterans and their families from different social and ethnic groups while communicating confidence in the veteran's ability and worth, helping the veteran address problems utilizing larger support systems as appropriate.
Provides consultation services to other VA staff regarding the psychosocial needs of homeless Veterans and the impact of psychosocial problems on health care and adherence to treatment plans.
Evaluates their own professional practice through peer review, case conferences and other organizational means.
Communicates effectively, both orally and in writing, with people from varied backgrounds.
Has knowledge of Veterans' benefits and services relating to special programs, service connected compensation and non-service connected pension.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Master's degree in Social Work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Note: A doctoral degree in Social Work may NOT be substituted for the master's degree in Social Work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
Licensed or certified by a state to independently practice social work at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure.
Grade Determinations: In addition to the basic requirements stated above, the following qualification criteria must be met for each grade. The candidate's qualifications must clearly demonstrate the level of competence required for the grade.
Senior Social Worker, GS-12
Experience/Education: The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty.
Licensure/Certification: Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure.
Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, the candidate must demonstrate all of the following KSAs:
Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management.
Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice.
Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes.
Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills.
Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.
Physical Requirements: Traveling throughout the medical center and the community is required, as is performing activities involving sitting, walking, and prolonged standing, bending and carrying such items as books, papers, and files. In carrying out responsibilities it may be necessary for the incumbent to travel into the community where he/she conducts interviews with the Veterans, their families, representatives of community health and welfare agencies and law enforcement agencies. The incumbent must possess current driver's license and drive a government vehicle in carrying out professional duties when deemed necessary.
