The incumbent serves as a member of Psychiatry and Mental Health Service at the Richard L. Roudebush VA Medical Center. The Richard L. Roudebush VA Medical Center is a 1a-High Complexity Facility, that serves over 67,000 unique patients and has 13 off site community clinics, in addition to the homeless team, the community based extended care programs and a 50 bed Domiciliary.

Duties

Administrative Program Duties: Serve as facility Subject Matter Expert (SME) and Point of Contact (POC) for matters related to acute suicide crisis and COMPACT ACT Ensures that COMPACT ACT information is disseminated to key stakeholders both internal and external Work in partnership with VAMC HBU, VBA, CBI and financial services Work in partnership with the Transitional Care Coordination Team Work in partnership with the Suicide Prevention Team Navigation between varying levels of care Advocacy for Veterans to ensure they do not receive bills for COMPACT ACT care A singular POC for Veterans, families and caregivers during COMPACT ACT episode of care Clinical/Case Management: Initiate contact with Veteran to discuss COMPACT ACT program involvement and expectations Face-to-face and/or phone contact to introduce COMPACT ACT to the Veteran, and to be the "face" of VA Psychiatry and Mental Health Service while inpatient at the Non-VA inpatient mental health facility. The goal being to optimize continuity of care from the non-VA inpatient mental health facility to the VA outpatient setting Discuss with the Veteran what to expect upon discharge and the reason for such frequent contact Completion and utilization of a Suicide Prevention Safety Plan while still admitted to the Non-VA inpatient facility, and reinforce, review, and revise with the Veteran, as needed Track follow through with mental health services after inpatient discharge Ensure Veteran safety by providing outreach after missed appointments, by phone. Home visits should be utilized if clinically warranted after veteran has missed follow up appointments, is unable to be reached by phone and/or family cannot be reached to verify veteran's safety Build therapeutic alliances with the Veteran and establishing rapport with the non-VA inpatient mental health facility staff/organization Establish and maintain communication between providers, Veteran, and care team to allow for opportunities for clarification and feedback. Coordination across the spectrum of services. Effective transitions and warm hand-offs Engage in explicit discussions with the Veteran (and caregiver if available) regarding warning symptoms or signs to monitor that may indicate that the Veteran's condition has worsened and who to contact in an emergency. Reduce non-VA inpatient facility readmissions and length of stay to support fiscal accountability and cost-savings Coach and counsel Veterans and families Serves as a role model for utilization of appropriate communication and conflict management techniques, effectively guiding and motivating team members Evaluate practice and COMPACT ACT implementation and processes for the Psychiatry and Mental Health Service and the Medical Center, based on established professional, community and regulatory standards and recommends/implements any changes to meet those standards Work Schedule: Full Time, Monday - Friday 7:30am-4:00pm Telework: Not Available Virtual: This is not a virtual position. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Licensure: Must be licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. In addition, candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Physical Requirements: ???????The physical demands of the position involve standard office requirements such as using the telephone, retrieving files, writing reports, data entry and maintenance of computer databases, etc. Work is typically performed sitting at a desk and using a computer. This position may require occasional travel and driving a government vehicle. The position requires periods of walking, standing, sitting, and bending.
