This position is in the Primary Care Service Line located at the VA Fayetteville Coastal Healthcare System within the Primary Care Service Line. The incumbent in this position serves as the Primary Care Management Module (PCMM) Coordinator for the Medical Center and the Community-Based Outpatient Clinics (CBOCs), and provides management of the PCMM software programs, all outpatient clinic profiles, and administrative support to the Patient Aligned Care Teams (PACTs).

Duties

These major duties include, but may not be limited to the following: Responsible for the integrity of the Medical Center's significantly large PCMM database; Monitoring and maintaining provider profiles, monitoring panel sizes, oversight and correction of transmission errors, and review/rectification of incorrect patient assignments. Develops and maintains specific reports used by the team to monitor, track and trend care delivery. Independently manage position responsibilities with little supervision as well as set priorities and deadlines adjusting the flow and sequencing of the work to meet team and patient needs; Collects, compiles, analyzes and presents data regarding the integrity of the PCMM package and uses this analysis to prepare and develop PCMM program evaluation reports; Gathers and maintains valid data used to develop, evaluate, and recommend modifications to structure and systems to achieve program goals. Prepares national, VISN and Medical Center level reports. Act as a resource to clinical staff in troubleshooting of VISTA and PCMM software applications associated with the Primary Care Clinic treatment area. Perform work in direct support of an administrative area such as: management analysis or operating program analysis for the Primary Care Management Module package. Provide assistant or technical level support from within the Primary Care Service or a comparable setting. Oversight of all PCMM activities and is able to make judgments without direct supervision on panel changes, provider assignments and assist patients with issues staying within established guidelines. Enter Primary Care team information into PCMM system, assign patients into the Primary Care teams, assign patients to appropriate provider within PCMM systems. Perform work supporting the study of organizations, including work related to surveys conducted to assess organizational performance for the Primary Care Service, Primary Care teams and the PCMM. Ensure all duties are completed regarding PCMM implementation and utilization. Maintain full responsibility for PCMM in the Primary Care setting. To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 10/24/2023. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-09 position you must have served 52 weeks at the GS-07. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment. You may qualify based on your experience and/or education as described below: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-07 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: assisting with administrative issues; planning and preparing for meetings including scheduling, mailings, travel and lodging arrangements; maintaining calendars; providing agendas; preparing responses to correspondence; scheduling appointments and meetings; answering phones; tracking time and attendance; and maintaining all paper and electronic files. OR, Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have a master's or equivalent graduate degree or at least 2 full years of progressively higher-level graduate education leading to such a degree. Physical Requirements: The work is mostly sedentary. The work requires some walking, standing, bending, lifting and carrying light items such as paper, books, and charts. Work Environment: The work is performed in an office setting. The employee is expected to follow safe work habits while operating office equipment.
