This position is located at Gallup Indian Medical Center(GIMC), Diabetes Program, Gallup, NM. The purpose of the position is to provide nursing expertise and leadership in assessing, planning, implementing, and evaluating care provided to individuals with or at risk for Type 2 Diabetes Mellitus. Provides diabetes training and consultation to service unit, tribal, area and IHS-wide; community-based diabetes intervention and prevention on case management and diabetes care.

Manage care of clients diagnosed with or at risk for Type 2 Diabetes Mellitus; utilizing professional judgment and the IHS Diabetes Standards of Care
Teaches the pathophysiology of diabetes, diet/exercise, medications, prevention and management of acute and chronic complications, self-care and management of diabetes
Provides individual and group education Evaluates the psychosocial and cultural aspects of diabetes as a chronic disease
Participates in the diabetes program with multidisciplinary team to assess and provide culturally relevant care for patients living with diabetes.
Performs clinical procedures when necessary
Coordinates diabetes care and patient visits with other departments for the prevention of diabetes-related complications Works closely with other members of the Diabetes Team

Qualifications
To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying.

BASIC REQUIREMENT(S):
Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing. A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education , Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
Experience: The work experience must have equipped the applicant with the particular knowledge, skills, and abilities to successfully perform the duties of the position. At the GS-09 and above grade level, many positions may require experience in a specialty area of nursing.
Basic Requirements for the GS-11 (or equivalent) Grade Level In addition to the above requirements, applicants must meet one of the following requirements:
Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field; or
At least one full year of professional nursing experience (equivalent to the next lower grade level) and possession of a diploma, associate degree, bachelor's degree, or maser's degree from a professional nursing educational program.
In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below.
MINIMUM QUALIFICATIONS:
GS-11: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Plan and carry out assignments, assess diabetes education and status of Diabetes Standards of Care, discuss treatment plan with physicians. Coordinate care with other departments. Work with patients in the specialty of diabetes treatment and prevention in a community or clinical setting.
Current licensure as a Registered Nurse is required; certification as a Diabetes Educator (CDE) is preferred.
